Endovascular repair of thoracic aortic pathologies: Postoperative nursing implications
Endovascular stent grafting is increasingly used to manage descending thoracic aortic pathologies. The procedure was introduced at the study hospital in 2001. We sought to examine the short-term clinical outcomes of patients who underwent this endovascular stent grafting, with the aim of using the result as baseline for development of an in-center clinical management protocol. We undertook a single-center, retrospective review of health care records of patients managed with thoracic stent grafts from 2001 to 2009. Patient characteristics, in-hospital data, and procedural data were obtained. SPSS was used to analyze the dat...

Demographic profile, clinical characteristics and outcomes of patients undergoing coronary artery bypass grafting—retrospective analysis of 4,024 patients
Conclusion Compared to Western population, this cohort of patients who underwent coronary artery bypass grafting had higher incidence of diabetes mellitus and ischemic cardiomyopathy and low incidence of significant left main disease. Post-operative stroke rate was significantly lower compared to 1–2 % stroke rate reported in western population.

Individualized antithrombotic therapy.
Conclusion: An extensive therapeutic arsenal to interfere with clot formation requires an individualized approach considering the disease condition and co-morbidities of the patient, the anticoagulants' and patientcharacteristics. This review builds on and extens previous publications of the authors on this topic. PMID: 25597592 [PubMed - as supplied by publisher]

Blood Pressure Lowering in Type 2 Diabetes A Systematic Review and Meta-analysis
ImportanceLowering blood pressure (BP) is widely used to reduce vascular risk in individuals with diabetes.ObjectiveTo determine the associations between BP–lowering treatment and vascular disease in type 2 diabetes.Data Sources and Study SelectionWe searched MEDLINE for large-scale randomized controlled trials of BP–lowering treatment including patients with diabetes, published between January 1966 and October 2014.Data Extraction and SynthesisTwo reviewers independently extracted study characteristics and vascular outcome data. Estimates were stratified by baseline BP and achieved BP, and pooled using fixed-effects m...

Coronary Artery Bypass Grafting After Percutaneous Intervention Has Higher Early Mortality: A Meta-Analysis
Conclusions Patients undergoing CABG after prior percutaneous therapy have a higher incidence of myocardial infarction and mortality in the postoperative period. However, midterm survival is comparable in both cohorts.

Predictors of acute kidney injury after coronary artery surgery in Jordanians
Conclusion Emergency coronary surgery, mitral regurgitation, prolonged inotropic support, and stroke or transient ischemic attack are independent predictors of acute kidney injury following coronary bypass surgery. Further studies involving patients with isolated coronary bypass surgery as well as valve and combined surgeries may be necessary to complete our understanding of this subject.

The Midterm Impact of Transcatheter Aortic Valve Replacement on Surgical Aortic Valve Replacement in Michigan
Conclusions TAVR implementation in Michigan has dramatically increased overall SAVR volume. This phenomenon has occurred with a concomitant decrease in preoperative risk profile and has improved early SAVR outcomes, particularly at TAVR hospitals, but surprisingly not in patients considered at high preoperative risk. As TAVR use increases, these issues may be further clarified and elucidated.

Hypertension in pregnancy and future maternal health.
Abstract Preeclampsia is a specific risk factor chronic of hypertension, of coronary events (Ray et al., 2005), of stroke (Brown et al., 2006), of chronic renal failure (Kattah and Garovic, 2005; McDonald et al., 2003; Williams, 2003) and a specific risk factor of cardio-cerebrovascular mortality (Magnussen et al., 2009). According to Magnusson et al. (2009), the relative long-term risk of developing hypertension is multiplied by 4; the risk of diabetes and metabolic syndrome multiplied by 3; the risk of coronary heart disease and stroke multiplied by 2. Cardiovascular risk is particularly important when pregnancy...

Direct oral anticoagulant use and the incidence of bleeding in the very elderly with atrial fibrillation
AbstractAtrial fibrillation (AF) is a major risk factor for stroke in the elderly population. The use of anticoagulation in patients with AF greatly reduces the risk for stroke, but results in an increased risk of bleeding. Over the past several years, direct oral anticoagulants (DOACs, dabigatran, rivaroxaban, and apixaban) have been used in place of warfarin for stroke prevention in AF. We conducted a retrospective cohort study to assess the safety of DOACs in very elderly patients (75+) managed in a health care system encompassing both community and academic settings. We found that 36  % of patients had moderate to sev...

Outcomes in Post ‐Myocardial Infarction patients similar to those of the PEGASUS‐TIMI 54 trial: a cohort study in the French national claims database
ConclusionsPatients selected using the criteria described in PEGASUS were older with more comorbidities, resulting in higher all‐cause death bleeding rates, but similar MI recurrence rates.

Trends in Hospitalization for Hypertensive Emergency, and Relationship of End-Organ Damage With In-Hospital Mortality.
CONCLUSION: A rising trend in hospitalization for HTNE, with an overall decrease in in-hospital mortality was observed from 2002 to 2012, possibly related to changes in coding practices and improved management. Presence of acute cardiorespiratory failure, stroke/TIA, chest pain, and aortic dissection were most predictive of higher hospital mortality. PMID: 28430850 [PubMed - as supplied by publisher]
