Branch-first continuous perfusion aortic arch replacement. Insight into our results
CONCLUSIONS: BF-CPAR is safe and adds to the armamentarium of Aortic Arch Repair. In elective and uncomplicated acute-dissection cases, it has no mortality and low stroke (1.9%), and vital organ dysfunction risk. Its results which are comparable to many of the best currently reported series, is driven by avoidance of cerebral circulatory arrest and reduction of cardiac and visceral ischemic time.PMID:35238522 | DOI:10.23736/S0021-9509.22.12272-X

Transapical aortic valve replacement in extreme-risk patients: outcome, risk factors and mid-term results ADULT CARDIAC
CONCLUSIONS TA-TAVR in extreme-risk patients carries a moderate risk of hospital mortality. Severe comorbidities and presence of residual paravalvular leakages affect the mid-term survival, whereas surviving patients have an acceptable quality of life without rehospitalizations for cardiac decompensation.

Outcomes of Heart Transplantation in Small Children Bridged With Ventricular Assist Devices.
CONCLUSIONS: Small children bridged to HTx with a VAD have early survival rates that are comparable to nonbridged children; however, this is achieved at the expense of a higher rate of stroke. Identifying the risk factors for early death and stroke in small children bridged to HTx with VADs is prudent as more experience with this patient population accumulates. Antithrombotic therapy in patients with atrial fibrillation and acute coronary syndrome in the real world: data from the Berlin AFibACS Registry.
CONCLUSIONS: These data suggest that in AFibACS patients, antithrombotic treatment focussed on dual antiplatelet therapy for ACS, rather than on anticoagulation therapy for stroke prevention. Factors influencing therapy at discharge were age, renal failure, stent implantation, AFib category, and CHA₂DS₂-VASc score. During the study period, triple therapy increased for stented patients. Blood pressure targets for hypertension in people with diabetes mellitus.
CONCLUSIONS: At the present time, evidence from randomized trials does not support blood pressure targets lower than the standard targets in people with elevated blood pressure and diabetes. More randomized controlled trials are needed, with future trials reporting total mortality, total serious adverse events as well as cardiovascular and renal events. Nationwide Comparative Impact of Thoracic Endovascular Aortic Repair of Acute Uncomplicated Type B Aortic Dissections
Conclusions: Thoracic endovascular aortic repair for acute uncomplicated TBAD was associated with similar in-hospital mortality, MI, and renal failure as compared to medical management. The TEVAR had higher rate of stroke up to the age 70 years and longer LOS. Because extending TEVAR to less complicated patients could only decrease TEVAR mortality rates, these findings support the more widespread use of TEVAR to treat patients with uncomplicated TBAD.
