Management of Large Vessel Occlusion Stroke Related to Infective Endocarditis: Is Mechanical Thrombectomy a Safe Option?
Acute ischemic stroke is the most common neurological complication of infective endocarditis. Intravenous thrombolysis is contraindicated in these patients due to a higher risk of hemorrhagic complications. Whether mechanical thrombectomy has some benefit in these patients remains unanswered although some favorable results can be found in literature.

Diagnostic Performances of the Susceptibility Vessel Sign on MRI for the Prediction of Macroscopic Thrombi Features in Acute Ischemic Stroke
In 20% –40% of cases the etiology of acute ischemic stroke (AIS) remains uncertain despite complete diagnostic investigation.1 Correlations between the histological microscopic features of the thrombus and the TOAST (Trial of Org 10 172 in Acute Stroke Treatment) categories have been found.2–5 However, from the macroscopic point of view, only one case report has considered the chromatic visual aspect of the retrieved thrombus.6 Recently, the “white” visual aspect of the thrombus at the moment of withdrawal has been related to atypical etiologies, in particular infective endocarditis.

Aortic and mitral valve surgery for infective endocarditis with reconstruction of the intervalvular fibrous body: an analysis of clinical outcomes.
Conclusions: The double-valve replacement and AMC reconstruction (the Commando procedure) is an effective technique in complex heart valve disease. Anticipating the management of Streptococcus agalactiae infective endocarditis
A patient who presented after falling out of bed had signs of right-sided stroke, which, after imaging, was suspicious for embolism with cardiac origin. Further investigation revealed a large aortic valve vegetation with mild aortic insufficiency, and the patient eventually developed heart failure requiring surgical intervention. A Complicated Case of Haemophilus influenzae Endocarditis in an Adult
Bacterial causes for infective endocarditis have changed over the years, with Staphylococcus aureus now being the most common pathogen isolated. With the advent of routine childhood vaccinations, invasive infections caused by Haemophilus influenzae have become rare. We wish to report a case of infective endocarditis caused by H. influenzae that was complicated by meningitis, septic arthritis, and stroke.

Efficacy of primary surgical versus medical intervention for treatment of left-sided infective endocarditis.

Outcome of surgery for acute infective endocarditis: does preoperative stroke have an impact on mortality?
ConclusionSurgical treatment for IE still remains a challenge with high morbidity and mortality. Patients with preoperative neurologic dysfunction due to septic embolism have good early postoperative results without increased mortality.

Safety and Efficacy of Thrombolysis and Mechanical Thrombectomy in Infective Endocarditis
We read with much interest the article “Acute Ischemic Stroke Treatment in Infective Endocarditis: Systemic Review” by Bettencourt et al, published in your esteemed journal. Authors have reviewed 37 articles including 52 patients. Sixteen of whom received IV thrombolysis, 22 received mechanical thrombectomy, 11 received combined trea tment. The authors stated that the quality of the articles were low. And have concluded that the risk of intracerebral hemorrhage was 4 times higher in the group treated with IV thrombolysis comparing to the group treated with thrombectomy.

We thank you for your interest in our recent systematic review and share your view that the efficacy results of thrombolysis and mechanical thrombectomy in infective endocarditis should be read with caution, because of the low quality of available evidence, as shown in our review.

Baseline frailty status and outcomes important for shared decision-making in older adults receiving transcatheter aortic valve implantation, a prospective observational study.
CONCLUSIONS: TAVI patients had improvement in symptoms and maintenance of activity of daily living at 6 months. They had low mortality and most patients lived in their own home 2 years after TAVI. Complications like death, stroke, and endocarditis occurred. Some patients had cognitive impairment before the procedure which might influence decision-making. Comparison Between Healthcare-Associated and Community-Acquired Infective Endocarditis at Tertiary Care Hospitals in Japan.
CONCLUSIONS: HAIE has different clinical characteristics and causative microorganisms, as well as worse prognosis, than CIE.
