Neurocritical Care Aspects of Ischemic Stroke Management
This article reviews the care of patients with ischemic stroke in the intensive care unit, including early general critical care interventions for airway control blood pressure goals according to the type of acute stroke treatment, poststroke cerebral edema management, hemorrhagic conversion in ischemic stroke, fibrinolytic reversal, and management of carotid endarterectomy and infective endocarditis. The importance of preventing common intensive care complications is discussed, including aspiration pneumonia, deep venous thrombosis, urinary tract infections, cardiac arrhythmias, and hyperglycemia.

Contemporary demographics, diagnostics and outcomes in non-bacterial thrombotic endocarditis
Conclusions NBTE was more prevalent in females and embolic complications were the most frequent clinical finding. Overall, patients with NBTE had a poor prognosis, particularly in those with lung cancer or metastatic tumours. Further studies in patients with NBTE are needed given its morbidity and mortality.

Temporal trends in incidence, patient characteristics, microbiology and in-hospital mortality in patients with infective endocarditis: a contemporary analysis of 86,469 cases between 2007 and 2019
ConclusionsIn this contemporary cohort, incidence of IE increased over time and in-hospital mortality remained high (~  15%). Whilestaphylococcus andstreptococcus were the predominant microorganisms, bacteremia withstaphylococcus and Gram-negative pathogens were associated with higher likelihood of in-hospital mortality. ConclusionA considerable challenge for physicians is evaluating all the signs suggestive of embolic sources in acute stroke and identifying the primary etiology when there are multiple causes. Early diagnosis and surgical intervention for bicuspid aortic valve (BAV) vegetation complicated by acute stroke may yield favorable clinical results.

ConclusionCryptogenic strokes in patients with cancer are cardioembolic manifestations of cancer-mediated hypercoagulability such as NBTE, which consists of sterile, platelet-fibrin vegetations on cardiac valves. The mechanism of thromboembolism in patients with cancer is based on the interaction between tumor cells and leukocytes, which creates a thrombogenic vascular lining and induces the release of substances that activate the coagulation cascade. The diagnosis of NBTE should depend on the distribution of occluded vessels, identification of thrombus, and exclusion of other possible causes such as infective endocarditis...
