What Are the Types of Tracheoesophageal Fistulas?
Discussion Respiratory problems in infants can caused by many things including any type of obstruction from the nose to lung. Neonates and infants are particularly susceptible to changes in the size of the airway because of the physics of airflow resistance. Important reminders about infant airways: Resistance = 1/radius4 (Poiseuille’s equation) therefore even a 1 mm decrease in the airway circumferences increases the airflow resistance x16. Increased airflow causes less pressure along the walls, which can lead to collapse of the walls (Bernoulli’s and Venturi effects). Neonates have smaller lungs relative to ...

Reliability of the supraorbital ethmoid cell vs Keros classification in predicting the course of the anterior ethmoid artery
ConclusionThe presence of SOEC is associated with a higher prevalence of the AEA coursing below the level of the skull base in all Keros types, thus placing the artery at greater risk for injury. Careful surgical planning is needed to avoid potential orbital complications.

Radiographic Nuances in Allergic Fungal Rhinosinusitis.
CONCLUSION: In this cohort, pediatric AFRS patients had more unilateral disease and anterior ethmoid skull base erosion. Concha bullosa prevalence was significantly higher in AFRS as compared to those with CRSwNP or CRSsNP, despite prior ESS. Surgeons should consider concha bullosa as a potential anatomical subsite to harbor recurrent or residual disease. PMID: 30674195 [PubMed - as supplied by publisher]

Efficacy and safety of burosumab in children aged 1–4 years with X-linked hypophosphataemia: a multicentre, open-label, phase 2 trial
Publication date: Available online 9 January 2019Source: The Lancet Diabetes & EndocrinologyAuthor(s): Michael P Whyte, Thomas O Carpenter, Gary S Gottesman, Meng Mao, Alison Skrinar, Javier San Martin, Erik A ImelSummaryBackgroundChildren with X-linked hypophosphataemia have high concentrations of circulating phosphatonin fibroblast growth factor 23 (FGF23), which causes renal phosphate wasting and hypophosphataemia, rickets, skeletal deformities, and growth impairment. Burosumab, a human monoclonal antibody against FGF23, improves phosphate homoeostasis and rickets in children aged 5–12 years with X-linked hypophosphat...

What Are The Potential Complications of a Tracheostomy?
Discussion In the past, indications for tracheostomy were often because of infection, but today are more often due to need for chronic airway support and/or ventilation. Many more children and infants are having tracheostomies placed. Decannulation rates for these children are low due to concomitant complex chronic disease such as neurological, pulmonary and congenital heart diseases. Co-morbidies are common with 3-4 being average, and frequently also including chronic technology such as gastrostomy tubes or ventriculoperitoneal shunts. Approximately 8% of children do not survive the hospitalization where the tracheostomy ...

Co-existent TBX1 Mutation and Chromosomal 20q13.13-q13.2 Duplication in an Infant with Abnormal TREC Newborn Screening
An eight day old male, term infant of a diabetic mother with patent ductus arteriosus was transferred from an outlying facility for surgical management of truncus arteriosus (TA), coarctation of the aorta, hypoplastic aortic arch, and anomalous origin of the right subclavian artery. The newborn also had absent thymic shadow on chest radiograph, hypocalcemia, and abnormal newborn screening (NBS) for severe combined immunodeficiency (SCID) measuring T cell receptor rearrangement circles (TRECs); threshold cycle (Ct) was 36.86 (Normal
