Use of intra ‐operative frontal sinus mometasone‐eluting stents decreased IL‐5 and IL‐13 in patients with chronic rhinosinusitis with nasal polyps
Conclusion: Although patients who received intra-operative frontal MES had significantly higher pre-ESS MM IL-13 and ECP, patients who received frontal MES had lower concentrations of IL-4 and IL-13 than those who did not at a median 8 months post-ESS. However, these changes did not correspond to significantly different measures of symptomatic or radiographic disease severity. This article is protected by copyright. All rights reserved

Fatal noncardiogenic pulmonary edema related to nonionic, iso-osmolar iodine contrast medium: one case report
Noncardiogenic pulmonary edema (NCPE) is a rare and life-threatening allergy-like reaction to the intravascular injection of a nonionic radiographic agent. We first describe a very rare case of fatal NCPE afte...

What Causes Rectal Prolapse?
Discussion Rectal prolapse is defined as a herniation of the rectum through the anal verge. In children it is also usually mucosal prolapse and not full thickness. Partial or mucosal prolapse usually is seen as radial folds occurring 1 – 2.5 cm from the anal verge. Full thickness will protrude more than 5 cm. It affects genders equally and occurs most commonly in children under age 4. This is due to vertical course and low position of the anus, looser supporting tissues and less muscular support. These children are more likely to have prolapse but are less likely to have it recur. After age 4 the rectum takes the adu...

Dupilumab improves objective, subjective, and health-related quality of life outcomes in chronic rhinosinusitis with nasal polyps (CRSwNP), regardless of BMI ≥30 kg/m2 or weight ≥90 kg: post-hoc analysis of the SINUS-24 and SINUS-52 studies
Dupilumab 300 mg q2w significantly improved endoscopic, radiographic, clinical, and patient-reported outcomes as well as health-related quality of life in patients with CRSwNP in the SINUS-24 and SINUS-52 studies (NCT02912468/NCT02898454). This post-hoc analysis investigated dupilumab ’s efficacy in the SINUS studies according to baseline body mass index (BMI) and weight.

IFN- γ Levels Prior to Endoscopic Sinus Surgery are Correlated with Post-Surgical Patient Reported Outcome Measures in Patients with Chronic Rhinosinusitis with Nasal Polyps
Chronic rhinosinusitis with nasal polyps (CRSwNP) is best characterized by type 2 (T2) inflammation but may have a mixed inflammatory endotype that includes type 1 (T1) inflammation. In prior analysis, we found that T2 markers correlated with cross-sectional outcomes but did not predict intermediate-term outcomes after endoscopic sinus surgery (ESS). Here we explore how IFN- γ, a classic T1 cytokine, correlates with symptoms and radiographic findings both before and after ESS.

Oral corticosteroid use and the risk of developing avascular necrosis: A large retrospective review
ConclusionFor patients receiving low cumulative doses of oral corticosteroids, corticosteroids pose a small risk of development of AVN. More studies are required to better characterize risk.

Immunoglobulin G and subclasses in COPD exacerbator phenotype: levels in exacerbation time and clinical stability
Conclusions: Patients with an exacerbator phenotype have lower levels of IgG. Both groups have lower levels in exacerbation time and its recovered in clinical stability.Clinical Implications These results could open a new path by considering the administration of Ig as a possible treatment in these patients to prevent exacerbations.

M122 mepolizumab: a steroid alternative for chronic eosinophilic pneumonia
Chronic eosinophilic pneumonia (CEP) is an idiopathic inflammatory disease characterized by pulmonary eosinophilia. Patients can present with progressive respiratory symptoms including dyspnea, productive cough, fever, and opacities seen on radiography. Two third of patients can present with concomitant asthma. Despite favorable therapeutic response to systemic corticosteroids, relapses upon cessation of therapy or tapering may necessitate prolonged treatment.

Efficacy of interferon beta-1a plus remdesivir compared with remdesivir alone in hospitalised adults with COVID-19: a double-bind, randomised, placebo-controlled, phase 3 trial
Lancet Respir Med. 2021 Oct 18:S2213-2600(21)00384-2. doi: 10.1016/S2213-2600(21)00384-2. Online ahead of print.ABSTRACTBACKGROUND: Functional impairment of interferon, a natural antiviral component of the immune system, is associated with the pathogenesis and severity of COVID-19. We aimed to compare the efficacy of interferon beta-1a in combination with remdesivir compared with remdesivir alone in hospitalised patients with COVID-19.METHODS: We did a double-blind, randomised, placebo-controlled trial at 63 hospitals across five countries (Japan, Mexico, Singapore, South Korea, and the USA). Eligible patients were hospita...
