Stevens Johnson Syndrome versus Jarisch Herxheimer Reaction in an HIV positive patient with secondary syphilis.
CONCLUSIONS: although the diagnosis of SJS has established criteria, the possibility of overlap with infectious illness and/or with its treatment, may complicate the diagnosis. PMID: 29076438 [PubMed - as supplied by publisher]

Adenoidal hypertrophy and allergic rhinitis: Is there an inverse relationship?
CONCLUSION: This real-life study shows that large adenoids may be associated with absence of allergy, whereas large turbinates may be associated with small adenoids. PMID: 23406587 [PubMed - in process]

Chapter 9: Benign sinonasal neoplasms.
Abstract Benign sinonasal neoplasms are a heterogeneous group of tumors that present with similar symptoms including nasal obstruction, anosmia, rhinorrhea, and epistaxis. The proper workup and accurate diagnosis is essential for these tumors so that the appropriate treatment plan can be established. In this article of benign sinonasal neoplasms, we discuss their typical clinical presentation, histological and radiographic findings, and treatment options. PMID: 23711037 [PubMed - in process]

Chapter 7: Allergic fungal rhinosinusitis.
Abstract Allergic fungal rhinosinusitis (AFRS) is a type of chronic rhinosinusitis in which patients classically exhibit nasal polyps, type I IgE-mediated hypersensitivity, characteristic findings on computed tomography scans, eosinophilic mucin, and positive fungal stain. New research has sought to further understand the pathophysiology of AFRS. However, this has also led to debate about the classification and predominance of this interesting disease process. Historically, patients with AFRS are immunocompetent. The disease is most prevalent in the southeast and south central United States and typically presents ...

Systematic review of immunotherapy for chronic rhinosinusitis.
Conclusions are limited by the paucity of available data on the efficacy of IT for treating CRS-specific outcome measures. There is weak evidence to support the use of IT as an adjunctive treatment in CRS patients, particularly in the postoperative period. PMID: 24717953 [PubMed - in process]

Osteitis is associated with P-glycoprotein overexpression in patients with chronic sinusitis without nasal polyps.
CONCLUSION: Increased osteitis burden as measured by either the KOS or the GOS is associated with increased P-gp membranous expression in CRS. Radiographic quantification of osteitis may therefore be used to identify patients with P-gp overexpression thereby providing a novel potential therapeutic target. Schwannomas of the sinonasal tract and anterior skull base: A systematic review of 94 cases.
CONCLUSION: ASB and sinonasal schwannomas are rare lesions and should be considered in the differential diagnosis of ASB masses involving the cribriform plate with sinonasal extension. Gross total resection of these lesions should be considered the goal of operative management. PMID: 24717879 [PubMed - in process]

Regional and socioeconomic disparities in emergency department use of radiographic imaging for acute pediatric sinusitis.
CONCLUSION: The use of ED imaging in APS is appropriately associated with factors known to be associated with APS complications. However, additional disparities with respect to regional and socioeconomic factors exist. Interventions to eliminate these health care disparities in use of imaging resources may lead to quality improvement in care and outcomes for APS. PMID: 24717874 [PubMed - in process]

A case of idiopathic hypereosinophilic syndrome presenting with acute respiratory distress syndrome.
Authors: Lim KS, Ko J, Lee SS, Shin B, Choi DC, Lee BJ Abstract Although idiopathic hypereosinophilic syndrome(IHES) commonly involves the lung, it is rarely associated with acute respiratory distress syndrome (ARDS). Here we describe a case of IHES presented in conjunction with ARDS. A 37-year-old male visited the emergency department at Samsung Medical Center, Seoul, Korea, with a chief complaint of dyspnea. Blood tests showed profound peripheral eosinophilia and thrombocytopenia. Patchy areas of consolidation with ground-glass opacity were noticed in both lower lung zones on chest radiography. Rapid progression ...

State of the art: A systematic review of the surgical management of aspirin exacerbated respiratory disease.
CONCLUSION: The state of the art in the management of AERD patients suggests that surgery does play an important role in helping establish symptomatic control. In the future, more rigorous studies evaluating the comprehensiveness of surgery and postoperative adjuncts are required to understand their impact on long-term patient outcomes. Objective radiographic density measurements of sinus opacities are not strong predictors of noninvasive fungal disease.
CONCLUSION: Higher average HU more accurately predicts allergic fungal mucin whereas heterogeneity/high-density components more accurately predict fungal balls. No objective radiographic density measure, in isolation, is both sensitive and specific in predicting noninvasive fungal sinusitis. PMID: 25514484 [PubMed - in process]

Patterns and sequelae of sphenoid sinus fractures.
CONCLUSION: Nasal CSF leak was the most common sequela of sphenoid sinus fractures, whereas BCAI was extremely uncommon. Analysis of fracture patterns can be useful in determining the need for additional evaluation. PMID: 25975253 [PubMed - in process]

Higher antineutrophil cytoplasmic antibody (C-ANCA) titers are associated with increased overall healthcare use in patients with sinonasal manifestations of granulomatosis with polyangiitis (GPA).
CONCLUSION: Sinonasal GPA patients with presenting C-ANCA titers more than or equal to 1:80 demonstrated significantly greater overall healthcare use than their lower C-ANCA level counterparts (less than 1:80). However, no significant differences in otolaryngology resource use or LM scores were evident between the two titer groups. PMID: 25975251 [PubMed - in process]

The role of viruses in the clinical presentation of chronic rhinosinusitis.
CONCLUSION: Although traditionally difficult, advances in molecular sequencing enhance detection of viruses in the sinonasal tract. In this study, respiratory viruses were more commonly isolated from patients with CRS compared with healthy controls. Moreover, viral infection may play a greater role in symptom exacerbation in CRSsNP than in CRSwNP. These findings warrant further investigation into the role of the viral microbiome in CRS.
