Abstract The development of food allergy is an increasingly recognized form of morbidity after solid organ transplant. It occurs more commonly in liver transplant recipients, although it has also been reported in heart, lung, kidney, and intestinal transplants. Pediatric transplant recipients are more likely to develop symptoms compared to adults, and reports of frequency vary widely from 5% to 38% in pediatric liver transplant recipients. Food allergies can persist after myeloablative hematopoietic stem cell transplantation in dedicator of cytokinesis 8–deficient patients
Dedicator of cytokinesis 8 (DOCK8) deficiency is a highly morbid combined immunodeficiency that features recurrent sinopulmonary infections, viral skin infections, and severe food allergies.1,2 Hematopoietic stem cell transplantation (HSCT) cures infection susceptibility in patients with DOCK8 deficiency.3-7 Whether HSCT also cures food allergy has not been systematically examined in human subjects.8 To gain insight into the etiopathogenesis of food allergy and its potential treatment, we studied food allergy in 12 DOCK8-deficient patients who underwent HSCT at the National Institutes of Health (NIH) Clinical Center.

Impact of Prior Drug Allergies on the Risk, Clinical Features, and Outcomes of Idiosyncratic Drug-Induced Liver Injury in Adults
ConclusionA prior drug allergy history was not associated with a greater likelihood of developing DILI compared to other causes of acute liver injury. However, the probable DILI patients with a history of prior drug allergy tended to have less severe liver injury and clinical outcomes. The low rate of suspect drug documentation in the “Drug Allergy” section of EMR after a DILI episode is of concern and could lead to avoidable harm from inadvertent suspect drug re-challenge.
