Transaortic septal myectomy at the time of aortic valve replacement for severe aortic stenosis: a case series of 55 cases
ConclusionAVR with CSM is a simple technique that can be utilised in severe AS with ABSH. There does not appear to be an increase in mortality or incidence of iatrogenic VSDs. Importantly, CSM allows for the implantation of a larger aortic valve compared to measurements made before CSM. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Surgical rescue mitral valve procedure post percutaneous transluminal mitral commissurotomy —a single-center observational study
This study evaluates the outcomes of surgical rescue performed within 24  h of undergoing PTMC from 1 January 2013 to 31 December 2019. Out of 2259 PTMC patients, 22 patients (<  1%) required rescue mitral valve surgery. Out of 22 patients, 17 patients (77.27%) developed mitral regurgitation; five patients (22.7%) had cardiac tamponade. Mitral valve replacement was performed in 20 patients (90.9%), while two patients (9.1%) underwent mitral valve repair. Cardiac tamponade was secondary to injury of the left atrium (9.1%), left ventricle (4.5%), and right ventricle (9.1%). Concomitant tricuspid valve repair was done i...

Ewing ’s sarcoma of kidney extending into IVC and RA: resection using modified cannulation strategy for CPB
AbstractEwing ’s sarcoma of the kidney is a rare tumor. Although renal carcinomas are known to involve the inferior cava, extension of the tumor up to the right atrium is not common. In the majority of cases when the tumor extends into the infrahepatic part of the inferior vena cava, it can be removed from the abdominal approach. Few patients require the use of cardiopulmonary bypass for removal of the tumor in the inferior vena cava and right atrium. The management of patients requiring resection of kidney tumors and right atrial mass is more complicated and requires a team approach consisting of oncosu rgeons, cardiac ...

Ebstein ’s anomaly with total anomalous pulmonary venous connection in an adult
We report the first successful s urgical correction of both Ebstein’s anomaly and total anomalous pulmonary venous connection in an adult patient. Such complex scenarios may pose unique challenges in management which require a judicious approach. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

N-Butyl cyanoacrylate embolization of an extremely rare variant of sequestration complex — a high-flow left-to-left shunt between systemic artery and pulmonary vein
AbstractA 19-year-old female presented with hemoptysis. Computed tomography (CT) pulmonary angiography revealed aberrant vessels from descending thoracic aorta, draining into pulmonary veins (left-to-left shunt). She was managed by transcatheter embolization of the aberrant vessels using N-butyl cyanoacrylate (NBCA) with balloon occlusion. A systemic artery to pulmonary vein fistula is one of the least common congenital anomalies. Most of the reported cases have been managed by surgery. Only a few patients have been treated by transcatheter embolization, using coils or vascular plugs as the embolizing agents. To our knowle...

Institution of cardiopulmonary bypass in an awake patient for resection of isolated intrathoracic right subclavian artery aneurysm causing near total compression of the trachea
AbstractThe subclavian artery ’s intrathoracic segment is a rare peripheral artery aneurysm site. Common causes are atherosclerosis, trauma, vasculitis, and infection. Subclavian artery aneurysms have a higher propensity for rupture, thrombosis, embolization, and compression of surrounding structures, thus necessitating urgent surgical care. Herein, we describe a case of isolated intrathoracic right subclavian artery aneurysm presenting with orthopnea and hoarseness due to near total tracheal compression and recurrent laryngeal nerve compression. We also describe in detail the necessary preparation for and use of pre-ane...

Post-ductal coarctation of aorta aneurysm repair in adult via left antero-axillary thoracotomy with antegrade cerebral perfusion —a case series
We describe our experience in managing three such patients through antero-lateral thoracotomy with antegrade cerebral perfusion. We have found this technique helpful in treating aneurysms of the distal aortic arch or proximal descending thoracic aorta. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Impact of distension pressure on saphenous vein endothelial injury in coronary artery bypass grafting
ConclusionUnregulated distension pressure during LSVG preparation is associated with greater endothelial damage, as evidenced by histological and immunohistochemical analyses. The inverse relationship between syringe size and pressure underscores the importance of controlled distension. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Bartonella quintana pulmonary native valve endocarditis
AbstractBartonella quintana is a well-known cause of blood culture-negative endocarditis; however, pulmonary valve involvement is rare. The case of a 40-year-old African male who presented to the Emergency Department with chest pain, cardiac failure, and a 2-week history of fever is presented. Transoesophageal echocardiography confirmed an atrial septal defect, severe pulmonary insufficiency with large vegetations, severe mitral regurgitation due to anterior leaflet prolapse, and right ventricular dysfunction. Empirical antibiotic therapy was started, and urgent surgical intervention was decided. There were vegetations on ...

Surgical bypass of superior and inferior mesenteric arteries with Dacron graft in a patient with acute on chronic mesenteric ischaemia with atherosclerotic complete occlusion of the infrarenal aorta
AbstractA 67-year-old male presented to us with symptoms of postprandial abdominal pain for the last 3  months and was not able to take meals for the last month. The computed tomography (CT) angiogram showed complete occlusion in the coeliac artery (CA), superior mesenteric artery (SMA) and inferior mesenteric artery (IMA) with complete occlusion of infrarenal aorta and bilateral iliac arteries. Whi le waiting for elective surgical mesenteric bypass, patient suddenly developed signs of peritonitis for which emergency surgery was done using a Dacron bifurcated graft 12 × 6 mm size. Inflow was taken from the supra coe...

Implantation of short-term biventricular assist device (BiVAD) using the CentriMag ™ system: the Manchester technique
AbstractBiventricular assist devices (BiVADs) using the CentriMag ™ system are being used increasingly as a form of short-term mechanical circulatory support for the treatment of acute cardiogenic shock from any aetiology. They can be used as a bridge to decision, recovery or transplantation. BiVADs are associated with better clinical outcomes when compared to v eno-arterial (VA) extracorporeal membrane oxygenator (ECMO) systems. In this paper, we describe a safe and reproducible method of BiVAD implantation using the CentriMag™ system at our institution. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)
