Surgical revascularization strategies of CAD involving dual left anterior descending artery
AbstractDual left anterior descending artery (LAD) is not an uncommon coronary artery anomaly. Preoperative identification of this anomaly in patients with coronary artery disease is important to develop strategies to ensure complete revascularization of the LAD territory. From April 1996 through February 2022, more than 16,500 patients underwent isolated coronary artery bypass surgery (CABG) by our team. Retrospective review of the angiographic and operative records of these patients revealed 85 cases with stenosis of the dual LAD system. The incidence of dual LAD in patients undergoing CABG is 0.51%. Among the 85 patient...

Open surgical replacement of the descending aorta: single-center experience
ConclusionsThis study showed that the elective open surgical replacement of the descending aorta can be achieved with excellent outcomes in terms of perioperative mortality and morbidity. Prosthetic graft is not a problem with open surgical descending aortic replacement, even in the long term. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Delayed simultaneous aortoesophageal and aortobronchial fistulae post thoracic endovascular aortic repair (TEVAR)
AbstractAortoesophageal and aortobronchial fistulae after thoracic endovascular aortic repair (TEVAR) are rare and life-threatening conditions. No clear guidance exists in the literature for the optimal therapeutic management of such cases. This case demonstrates a delayed simultaneous aortoesophageal and aortobronchial fistulae treated conservatively with culture-guided antibiotic therapy and combined endovascular management. The role of VATS in the removal of intrathoracic foreign bodies — a systematic review
ConclusionVATS is a safe and effective method for the removal of intrathoracic FBs from various thoracic sites that are associated with a good clinical outcome — in both elective and emergency cases. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Left middle lobectomy in a patient with Kartagener ’s syndrome
AbstractThe middle lobe is usually a part of the right lung as it is tri-lobar in normal anatomy. In patients with Kartagener ’s syndrome, the middle lobe belongs to the left lung due to situs inversus. They are prone to develop bronchiectasis due to ciliary dyskinesia and bronchial anatomy of the middle lobe. We hereby report an interesting case of left middle lobectomy done in a patient with Kartagener’s syndrome wit h bronchiectasis. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Inter-arterial course of the left coronary artery with a commissural origin in a 40-year-old patient with tricuspid atresia post Fontan shunt
We report a case of an adult male with Tricuspid atresia post Fontan shunt in whom a rare  coronary anomaly was demonstrated on Computed Tomography angiography using endoluminal views and volume rendering techniques. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Postoperative care after left ventricular assist device implantation: considerations for the cardiac surgical intensivist
AbstractHeart failure is a leading cause of morbidity and mortality, the incidence of which is predicted to continue to increase as the population ages. Left ventricular assist devices (LVADs) in particular have emerged as important therapies for the support of patients with advanced heart failure needing short- or long-term mechanical circulatory support. With over 5000 implantations per year, LVADs are the most commonly used durable devices worldwide. In this article, we provide an overview of the intensive care management of patients with LVADs during the early post-implantation period. (Source: Indian Journal of Thorac...

Current practice of saphenous vein graft harvesting in coronary artery bypass graft surgery: an Indian survey
ConclusionThere is a significant variance in the process of vein harvesting, preservation, handling, and antiplatelet therapy protocols among various cardiac surgeons across India. Therefore, there is a need for standardization in the practice of vein harvesting. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

How to select a patient for LVAD
AbstractLeft ventricular assist device (LVAD) implantation leads to improvement in symptoms and survival in patients with advanced heart failure. An important factor in improving outcomes post-LVAD implantation is optimal preoperative patient selection and optimization. In this review, we highlight the latest on the evaluation of patients with advanced heart failure for LVAD candidacy, including discussion of patient selection, implantation timing, laboratory and other testing considerations, and the importance of psychosocial evaluation. Such thorough evaluation by multidisciplinary team can serve to improve the outcomes ...

Anomalous origin of the left coronary artery from the right sinus of Valsalva with separate ostia and inter-arterial course: MDCT angiography depiction
We describe a case of anomalous left coronary artery origin from the right coronary sinus with separate ostia of right and left coronary arteries detected by 128-slice computed tomography (CT) coronary angiography. Apart from separate ostia, the left coronary artery had inter-arterial course between the aorta and the pulmonary trunk, also called as a malignant course. Giant thymoma with complete superior vena cava obstruction
We present a case of a stage IVa thymoma surgically treated. The superior vena cava was completely occluded and, after resection, reconstruction was deemed unnecessary due to sufficient venous return through the azygos-inferior vena cava system. In our opinion, despite the morbidity of surgery in advanced thymomas, it should always be considered in expert hands. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)
