Post-intubation double tracheal stenosis
We present the case of a 43-year-old male with a history of prolonged mechanical ventilation, who presented to our center with progressive respiratory impairment. Computed tomography with three-dimensional reconstruction demonstrated complex tracheal anatomy with double-level stenosis. The patient underwent successful endoscopic procedure. Myositis ossificans of the chest wall in an 8-year-old boy: a case report of a diagnostic pitfall
We report a case of an 8-year-old male who presented with painful and progressively enlarging left-sided chest wall mass. The tumor showed close histo-morphological mimicry with osteosarcoma. Moreover, the characteristic radiographic findings of myositis ossificans were ab sent. The age of the patient and the absence of attachment to the rib helped exclude extra-skeletal and parosteal osteosarcomas, respectively. The patient was doing well 4 months after surgery. A natural systemic to pulmonary artery shunt
We report a case in which this subclavian artery was supplying the pulmonary circulation in a patient with tetralogy of Fallot (TOF) which effectively acted as a natural systemic to pulmonary artery shunt. The patient had good room air saturation and no features of vertebrobasilar insufficiency or left upper limb ischemia. She underwent a total correction of TOF and ligation of the aberrant left subclavian artery. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

A case report on umbilical artery catheter embolization in a neonate, and its surgical management
We describe a term neonate, in whom a 3.5-Fr umbilical artery catheter, accidentally broken and dislodged, was surgically retrieved through right common iliac arteriotomy, safely and completely. Direct surgical retrieval of umbilical catheters while avoiding attempts at local exploration maybe considered a first line in management, particularly if endovascular facilities are unavailable or inaccessible. Congenital agenesis of anterior leaflet of tricuspid valve
We report the case of a 10-year-old girl who presented with severe tricuspid regurgitation with annular dilatation and non-coapting leaflets. Intra-operatively, an incidental finding of complete agenesis of the anterior leaflet was observed. A tricuspid valve replacement was carried out; the patient made an uneventful recovery and remains well at follow-up. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Veno-venous extracorporeal membrane oxygenation for the treatment of respiratory compromise
AbstractExtracorporeal membrane oxygenation for the purpose of intervening upon profound cardiovascular or pulmonary compromise has proven to be a worthy intervention. Technological advancements have allowed this mode of therapy to become more effective and widespread. Veno-venous extracorporeal membrane oxygenation (VV-ECMO) is a commonly used strategy to help manage patients with pulmonary dysfunction refractory to traditional management methods. This review intends to focus upon common indications and the clinical considerations for the institution of VV-ECMO as well as some of its known complications. (Source: Indian J...

Early and midterm outcomes after off pump coronary artery bypass surgery
ConclusionOPCAB can safely be performed with satisfactory short and mid-term outcomes. Further corroborative studies from different regions of the country or a multi-center study will help to establish the suitability of the technique in Indian patients. Left ventricular mass regression after aortic valve replacement with the TTK Chitra ™ monoleaflet tilting disc valve
ConclusionSignificant LV mass regression occurred after AVR with the TTK Chitra ™ valve. The maximum reversal was found to be within the first 3 months after surgery with sustained beneficial improvement for the next 3 years. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Discontinuation of ECMO —a review with a note on Indian scenario
AbstractExtracorporeal membrane oxygenation (ECMO) has strikingly progressed over the last 20  years in the management of adult and pediatric severe respiratory and cardiac dysfunctions refractory to conventional management. In this review, we will discuss the weaning strategies of veno-venous and veno-arterial ECMO including the bridge to recovery and bridge to transplant along with post-E CMO care. We will also discuss the futility and the management of bridge to nowhere from Indian perspectives. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Four-year outcome of the tricuspid valve leaflet augmentation and ring annuloplasty in dextrocardia
AbstractA 62-year-old male was admitted to our hospital with worsening dyspnea. Chest X-ray revealed dextrocardia, and echocardiography revealed decreased right ventricular function, a severely dilated tricuspid annulus, a massive tricuspid, and trivial mitral regurgitation. Patch augmentation of the tricuspid leaflet with auto-pericardium and ring annuloplasty with a flat-type upside-down artificial ring to fit the dextrocardia heart was performed. Echocardiography 4  years after the operation showed trivial regurgitation. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Sandwich stent located in the iliac artery dislodged into the thoracic aorta and its remedy
AbstractThe sandwich stent technique is a commonly used method to preserve unilateral internal iliac artery flow when treating iliac artery aneurysm. In this case, covered stent grafts (Viabahn, Gore) were used to build the iliac limb of a sandwich stent. However, if Viabahn is released without long sheath protection, the trigger wire is easily wound on another stent, resulting in the Viabahn delivery system being inseparable from the stent. If we drag it forcefully, it would easily cause the entire sandwich stent graft to shift or even fall into the thoracic aorta. This complication is catastrophic. Here we report the cas...

Management of ground-glass opacities and sub-solid pulmonary nodules: a surgeon ’s perspective
AbstractThe regular use of high-resolution computed tomogram scans has led to an increase in the detection of asymptomatic ground-glass opacities and sub-solid nodules at an early stage. Different growth patterns of these lesions are making decision-making a real challenge. With growing experience and improving radiology interventions, management of these lesions is changing constantly. However, with variations in growth patterns and outcomes, immediate treatment options as well as follow-up surveillance and subsequent interventions can be confounding for the clinicians. This mini review describes algorithms for managing t...
