Selecting the appropriate patients for coronary artery bypass grafting in ischemic cardiomyopathy —importance of myocardial viability
AbstractPatients who undergo coronary artery bypass graft (CABG) surgery in ischemic cardiomyopathy have a survival advantage over medical therapy at 10  years. The survival advantage of CABG over medical therapy is due to its ability to reduce future myocardial infarction, and by conferring electrical stability. Dual ruptured sinus of Valsalva aneurysms
We report a rare case of an 18-year-old male with dual RSOV aneurysms who was managed successfully by patch closure via the transaortic approach and which to our knowledge has never been reported (“PubMed” and “Google Scholar” as the sea rch engine with dual, ruptured, sinus of Valsalva aneurysm as the MeSH words). Dual RSOV aneurysm in a single patient is an extremely rare condition which can lead to early cardiogenic shock and should be managed surgically at the earliest. EVOLUT Low-Risk Trial data: “We see what we want to see!”
AbstractIn the 4-year update of the EVOLUT Low-Risk Trial (NCT02701283), the authors highlight a non-significant improvement in the primary endpoint and a statistically significant, yet clinically irrelevant, enhancement in hemodynamics with transcatheter aortic valve replacement (TAVR) compared to surgical aortic valve replacement (SAVR). Overlooking the concurrent surgical procedures in SAVR patients, mortality rates remain similar at 4 years. Unaddressed is the substantial increase in permanent pacemaker implantations with TAVR and the fate of patients with paravalvular leaks. Dual origin of the left vertebral artery: a rare anatomic variation
We describe a case of a duplicated left vertebral artery. Computed tomography (CT) angiography examination of the head and neck vessels of a 63-year-old man with a history of imbalance was undertaken. Successful endovascular management of coral reef aortic occlusion
We present our experience with endovascular management of 10 patients with coral reef aorta. Uncontrolled hypertension, chronic renal disease, disabling claudication, and critical limb ischemia with tissue loss were the presenting symptoms. Seven patients had infrarenal aortic occlusion, and 3 had occlusion at renal and suprarenal aorta. Eight had involvement of the visceral vessels and 3 had renal artery stenosis.
