AbstractInfective endocarditis frequently spreads beyond the valve tissue, especially in the aortic location. Invasive endocarditis may lead to abscess formation or fistula, with substantial tissue loss. Here, the case of a 31-year-old male patient with destructive aortic and pulmonary valve endocarditis and a subaortic mural defect who underwent patch closure of the ventricular septal defect and aortic and pulmonary root replacement and right coronary artery bypass graft is presented. Giant tortuous right coronary artery to coronary sinus fistula with severe tricuspid regurgitation
We report a case of a giant tortuous right coronary artery to coronary sinus fistula with severe tricuspid regurgitation in a 78-year-old female. The initial working diagnosis was based on clinical manifestations and echocardiography, further supported with computed tomography coronary angiography. The fistula was surgically closed with satisfactory results. Pulmonary valve replacement and left pulmonary arterioplasty in repaired tetralogy of Fallot through left vertical axillary thoracotomy
AbstractDifferent surgical approaches have been described for pulmonary valve replacement in patients with pulmonary regurgitation post tetralogy of Fallot repair —repeat median sternotomy, left anterior or antero-lateral thoracotomy, and left posterolateral thoracotomy. Every approach has its merits and drawbacks. In this case report, we describe a technique of pulmonary valve replacement and left pulmonary arterioplasty through left vertical axillary thor acotomy. ConclusionCardiac catheterization procedures have become increasingly safe, but complications can still occur, for which the cardiac surgeon should be briefed in a Heart Team meeting before taking up such cases. Treatment of left main coronary artery thrombus during SARS-CoV-2: early surgical revascularization is key
We describe a 38-year-old male who developed an ST-elevation myocardial infarction secondary to a left main coronary artery thrombus after SARS-CoV-2. The patient failed anticoagulation and fibrinolysis and developed decompensated heart failure. Ultimately, the patient underwent surgical revascularization, which led to full recovery. This highlights the need for refinement in this population. We recommend that early surgical intervention be considered in patients with ST-elevation myocardial infarction secondary to left main coronary artery thrombus in the setting of SARS-CoV-2. Implantation of a mitral allograft into the tricuspid valve position in active infective endocarditis
We present a clinical case detailing the successful implantation of a fresh mitral allograft into the tricuspid position in a 37-year-old male patient diagnosed with active infective endocarditis of the tricuspid valve and a high risk of prosthetic endocarditis. The mitral allograft was anatomically implanted with annuloplasty using an expanded polytetrafluoroethylene band. Subsequently, the patient was discharged in satisfactory condition.
