Metastatic choriocarcinoma to the heart and lung with successful multimodal management
AbstractCardiac tumors continue to be one of the most uncommon primary tumors. Secondary cardiac tumors are rare and occur in difficult instances. A multidisciplinary team is required for diagnosis and management. Within this case study, we present a 24-year-old female patient who experienced dyspnea and was diagnosed as having metastatic heart and lung choriocarcinoma that was effectively managed with heart surgery and chemotherapy. Lung preservation in a case of complex rupture of right main bronchus and segmental bronchi injury with lung laceration
We present a case of successful bronchial repair with preservation of the entire lung in a complex bronchial injury. An 18-year-old male presented with pneumothorax and minimal dyspnea following blunt chest trauma and was managed with intercostal drainage and resuscitation. A review after 48  h showed a fallen lung sign with a Cerfolio 4C air leak. Computed tomography (CT) scan confirmed the right main bronchial injury. On exploration, a lacerated consolidated right upper lobe with dusky discoloration was noted. Longitudinal avulsion of 2/3rd of the anterior wall of the right main bron chus (RMB) at the level of the upper...

Diagnostic work-up and current management strategies for infective endocarditis in the pediatric population
AbstractInfective endocarditis (IE)  remains a serious disease that is associated with significant morbidity and mortality, and despite the significant advances that have been made in understanding the disease process in past decades, its incidence appears to be on the rise recently. Endocarditis in children is no longer a rare occur rence. This appeared to be related to a combination of the improved survival of children with congenital heart diseases (CHDs), increase use of intracardiac protheses, and catheter-related interventions. The American Heart Association (AHA) 2007 guidelines reduced the recommendations for use ...

Suprasternal approach for insertion of Impella 5.5 into the proximal right subclavian artery
AbstractThe Impella 5.5 (Abiomed) is a percutaneous, temporary left ventricular assist device (LVAD) that serves as an important method of treatment of acute cardiogenic shock refractory to medical management. The Impella 5.5 and 5.0 are commonly inserted through the right axillary artery; however, this may be limited by inadequate vessel diameter to accommodate the Impella and inadequate vessel quality. A central approach to Impella 5.5 incision has been described in the pediatric population, particularly via the innominate artery through a suprasternal and/or neck incision, with success. As an alternative to axillary Imp...

Thoracolithiasis: a rare pearl earring-like lesion
AbstractThoracolithiasis has been reported as one or more free-mobile bodies, and this free mobility in the pleural cavities could be a clue to the diagnosis. Here, we report a rare case of immobile thoracolithiasis anchored to the pleural fat like a pearl earring. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Analysis of preoperative and postoperative blood gas indices and airflow dynamics with tracheal stenosis undergoing tracheal reconstruction
The objective of the study was to investigate the impact of tracheal reconstruction in cases of tracheal stenosis on blood gas indices as well as airflow indices and whether arterial blood gas (ABG) can be a better surrogate of adequacy of tracheal reconstructi on. This was a retrospective observational study of 61 patients with tracheal stenosis between the ages of 21 and 65 years who underwent tracheal reconstruction. The preoperative and postoperative values of various blood gas indices like partial pressure of oxygen (pO2), partial pressure of carbon dioxide (pCO2), and potential of hydrogen (pH) and airflow indices l...

FibroScan as a surveillance marker post-cavopulmonary anastomosis for single-ventricle physiology
ConclusionWe have found that LSM via FibroScan can effectively be a surveillance or screening test for FALD. Serial FibroScans can be used to monitor the progress of liver disease. Raised LSM may also have a role in predicting the morbidity for completion of Fontan post-BD Glenn operation. Large-scale studies are needed to validate our findings. VATS thoracic duct clipping in post-CABG with chylothorax
We report a case of 49-year-old female who underwent CABG, and developed pleural effusion on post-operative day 2 which was milky in nature. Chylothorax was confirmed based on the biochemical analysis of the pleural fluid. As the medical line of management failed, video-assisted thoracoscopic surgery (VATS) was done and thoracic duct clipped on the right side. Close to the proximal portion of the left internal thoracic artery, disrupted tributaries of thoracic duct were noted and clipped. Rarity of the case and management is highlighted. Bronchial carcinoid: a neuroendocrine manifestation —presented as bronchospasm and hypertension during resection
AbstractBronchopulmonary carcinoid tumors are rare, slow-growing malignant neuroendocrine tumors. Early diagnosis is pivotal as surgical resection is the main stay of treatment. A 40-year-old female with typical bronchial carcinoid presented with complaints of breathlessness on mild to moderate exertion, intermittent productive cough, low-grade fever, and loss of appetite since 1  year. Right lobectomy was performed with regional lymph node resection; during resection of the tumor, manipulation led to a massive surge and fluctuations in blood pressure which were managed with deepening of the plane of anesthesia and admini...

Risk factors and outcomes for surgical repair of obstructed total anomalous pulmonary venous connection
ConclusionsDespite logistical challenges, reasonable surgical outcomes are attainable in obstructed TAPVC cases. Age less than 30 days and need for pre-operative ventilation were independent predictors of in-hospital mortality, while pulmonary venous obstruction in the post-operative period contributed to re-interventions.
