A simple transseptal approach versus the conventional left atrial approach for mitral valve procedures: a retrospective study
ConclusionThe simple transseptal approach through the midpoint of the fossa ovalis could be a safe and even favorable alternative to the traditional left atriotomy approach in certain difficult operative situations during mitral valve replacement surgery.Graphical abstract (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Delayed aorto-atrial fistula —a rare complication of tricuspid valve endocarditis
This report is significant because a rare surgical pathology is visualized clearly and provides an educational value to aid other clinicians in the recognition and management of this unusual diagnosis. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Subcutaneous emphysema, pneumomediastinum and pneumothorax in COVID-19 pneumonia —independent prognostic factors
AbstractThe ongoing coronavirus disease 2019 pandemic has created a substantial disease burden and morbidity. However, the development of subcutaneous emphysema, pneumomediastinum, and pneumothorax have been of rare occurrence and their significance in mortality has not been studied. In a retrospective single-institution observational study at a tertiary care centre in the northern part of India, we evaluated the occurrence of these complications and their relationship with mortality from 1 June 2020 to 30 November 2020. All coronavirus disease 2019 (COVID-19) patients developing subcutaneous emphysema, pneumomediastinum, ...

Minimal access aortic surgery
AbstractRecently, there have been significant advances in small incisions through right thoracic cavity including robotic surgery. We have been performing aortic surgery through a partial sternotomy for ascending, arch, and aortic root replacement. In this article, we would like to provide tips for small incision aortic surgery at our institution by showing surgical videos of two cases. Comparison of the outcomes between total arch replacement and nontotal arch replacement in patients with acute type A aortic dissection
ConclusionThe mortality rates in ATAAD patients with TAR and non-TAR were similar. However, the incidence of new permanent postoperative neurological deficit was significantly higher, and the length of hospital stay was longer in patients with TAR. TAR in ATAAD should be avoided especially in patients who have experienced preoperative CPR to abate risk of PND. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Sporadic right ventricle rhabdomyoma in an adult without tuberous sclerosis
AbstractCardiac rhabdomyoma is a rare benign tumor of the pediatric age group which usually regresses by the age of 2  years. Cardiac rhabdomyoma in adults is still a rarer disease. It is usually associated with tuberous sclerosis. Patients with cardiac rhabdomyoma will usually be asymptomatic and detected incidentally, or in suspicion to the signs and symptoms of tuberous sclerosis. Symptomatic patients present w ith palpitations or failure to thrive due to obstructive symptoms leading to cardiac failure. The authors present a rare case of right ventricular rhabdomyoma in an adult without tuberous sclerosis treated surgi...

Sutureless repair of left ventricular free wall rupture following acute myocardial infarction with cardiogenic shock
We report a case of blow-out-type left ventricular free wall rupture (LVFWR) after acute myocardial infarction, who presented with unstable hemodynamic condition in New York Heart Association (NYHA) functional class IV. Immediately, we performed a successful LVFWR repair with sutureless technique using a glue and expanded polytetrafluoroethylene patch on cardio-pulmonary bypass support. Postoperative period and recovery was uneventful. Over a period of 2-year follow-up, the patient is in NYHA class I and cardiac magnetic resonance imaging showed adequate left ventricular (LV) function and no evidence of LV aneurysm. (Sourc...

Open aortic arch surgery —how we do it
AbstractOpen aortic arch surgery is still challenging even in the present era with the Achilles ’ heel being the neurological complication. The variety of approaches and surgical techniques used to reconstruct the aortic arch, protecting the vital organs, the extent of aortic replacement, and the options of reimplanting the supra-aortic arch vessels indicate the challenges involved in the pr ocedure. Though open surgery still remains the ‘gold standard’, in the last two decades, endo-prostheses and stents have emerged as alternate options. Surgical outcomes of common arterial trunk repair beyond infancy
ConclusionRepair for common arterial trunk in patients presenting beyond 1  year of age is challenging; however, it can be done with satisfactory early and late outcomes in terms of mortality and reintervention. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Which is better for pediatric and adult cardiac surgery: del Nido or St. Thomas cardioplegia? A systematic review and meta-analysis
ConclusionIn both adults and pediatric cardiac surgery, del Nido cardioplegia helps in lowering cardiopulmonary bypass duration, defibrillation rates, and hospital stay, compared to St. Thomas solution. Among adults, del Nido cardioplegia lessens the aortic cross clamp times with no difference observed in all-cause mortality, intensive care unit stay, or mechanical ventilation. Trans-aortic DORV repair
We present a case of pure trans-aortic repair of DORV VSD pulmonary stenosis (PS) in an adult. A 20-year-old male with a known case of DORV, VSD, and PS presented with cyanosis. A pure trans-aortic repair of DORV was done. Complete trans-aortic DORV VSD PS repair is yet to be reported in the literature. The trans-aortic approach avoids a right atriotomy, right ventriculotomy, and injury to coronary arteries in cases of complicated tetralogy of Fallot and avoids injury to the tricuspid valve. This approach can be used in selected patients for intracardiac repair. The advantages are excellent visualization of the defects and...
