Thoracic lipoblastoma in a 7-month-old infant
We report a rare case of a thoracic lipoblastoma in a 7-month-old infant who presented with respiratory infection, dyspnea, and a right-sided chest wall swelling. Imaging revealed a predominantly fatty mass, and biopsy confirmed the diagnosis. A right thoracotomy was performed, resulting in subtotal excision due to the tumor ’s friability. Two months later, a recurrent tumor was detected and was successfully treated with total excision. This case emphasizes the importance of complete surgical resection and regular follow-up to prevent local recurrence of thoracic lipoblastoma. The total artificial heart: where have we been, where are we now, where are we going?
AbstractThe incidence and prevalence of end-stage heart failure continue to rise; however, the number of donor hearts available for transplantation continues to be limited. Therefore, alternatives to transplantation, such as the use of total artificial hearts (TAH), are necessary. The long and winding road to the development and implantation of the ideal TAH remains under construction. Although efforts have been ongoing for almost a century, researchers and clinicians continue to improve currently available TAHs and design and construct new models. With mortality and morbidity rates decreasing, particularly at high-volume ...

RVOT pseudoaneurysm post biventricular repair for tetralogy of Fallot with single pulmonary artery
AbstractPseudoaneurysm of the right ventricular outflow tract (RVOT), post repair for tetralogy of Fallot (TOF), is a rare occurrence with few cases reported in literature. TOF with single pulmonary artery is in itself a rare occurrence. RVOT pseudoaneurysm in a case of TOF with single pulmonary artery has not been reported to the best of our knowledge. RVOT pseudoaneurysm is a catastrophic complication which has very few symptoms and has to be picked up early to avoid dire consequences. We have reported such a rare occurrence to highlight the importance of looking out for such complications in rare presentations where ana...

Solitary endobronchial plasmacytoma —a rare differential of an endobronchial mass
AbstractDiagnostic dilemma, owing to the inconclusiveness of biopsy results, often leaves us with limited options to offer to the patients upfront, amongst the various armamentarium available. We hereby report a rare case of extramedullary plasmacytoma, whose diagnosis was established only on the final histopathology report with the aid of immunohistochemistry. A 50-year-old gentleman presented to our outpatient setup with computed tomography suggestive of a well-defined endobronchial mass occupying the left lower lobe bronchus. However, bronchoscopy and computed tomography –guided biopsies were inconclusive. After a rou...

Left handedness in cardiac surgery —boon or bane?
AbstractLeft-handed individuals consist of around 11% of the population and true ambidextrous consists of only 2 –3% of the total population. Almost a similar ratio is seen in medical profession also. Difficulties are obvious for left-handed cardiac surgeons in the operating room due to regular right-handed instruments, laterality, and positioning of the patients. In this article, we discuss about the proble ms and the potential solutions for left-handed cardiac surgeons. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Managing valvular pathology during LVAD implantation
AbstractSince the time of their invention, implantable continuous flow left ventricular assist devices (LVADs) have improved the quality of life and extended survival for patients with advanced heart failure. The decision surgeons and their physician colleagues make with these patients to undergo implantation must come with full understanding of the immediate, short-term, and long-term implications of such a life-changing procedure. The presence of pathology regarding the aortic, mitral, and tricuspid valves introduces particularly complex problems for the surgical treatment strategy. Concomitant valve repair or replacemen...

To touch or not to touch: unveiling the key to vein graft patency
AbstractThe Graft Patency Between No-Touch Vein Harvesting Technique and Conventional Approach in Coronary Artery Bypass Graft Surgery (PATENCY) trial, a large multicenter study, compared the no-touch (NT) technique with conventional vein harvesting in coronary artery bypass graft (CABG) surgery. The study included 2655 patients and assessed graft patency at 3  months and 1 year using computed tomography (CT) angiography. The NT group showed a significantly lower occlusion rate at 3 months (2.8% vs. 4.8%) as well as at 1 year (3.7% vs. 6.5%). Recurrence of angina was also lower in the NT group at 1 year. The study pro...

Cell sheet transplantation for ischemic heart disease: a systematic review
ConclusionCell sheet transplantation can safely improve LVEF and NYHA class in ischemic heart disease patients, even in very low ejection fraction patients with unsuccessful standard therapy before. Further studies with better patient inclusion, larger population, and long-term follow-up required to confirm these results. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Acute thrombosis of the lower limb arteries on the background of the XBB.1.5 ( “Kraken”) subvariant of omicron SARS-COV-2 — medical or surgical treatment?
ConclusionAnticoagulant therapy demonstrates several advantages over aggressive surgical management. However, probably due to the small size of the studied sample, significant differences were not obtained. Further study of this issue is required with the identification of the optimal reperfusion method in this complex cohort of patients. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Open arch —my way! Branch first continuous perfusion arch repair (BF-CPAR)
AbstractWe herein describe our technique of “branch first continuous perfusion arch repair (BF-CPAR)” which does away with both cerebral circulatory arrest and the need for deep hypothermia. We use this technique for all aortic surgeries including for type A acute aortic dissections. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)
