Current investigative modalities for detecting and staging lung cancers: a comprehensive summary
AbstractThis narrative review compares the advantages and drawbacks of imaging and other investigation modalities which currently assist with lung cancer diagnosis and staging, as well as those which are not routinely indicated for this. We examine plain film radiography, computed tomography (CT) (alone, as well as in conjunction with positron emission tomography (PET)), magnetic resonance imaging (MRI), ultrasound, and newer techniques such as image-guided bronchoscopy (IGB) and robotic bronchoscopy (RB). While a chest X-ray is the first-line imaging investigation in patients presenting with symptoms suggestive of lung ca...

Primary pleuro-pulmonary Ewing ’s sarcoma: multimodal approach with uniportal thoracoscopic resection
AbstractPrimitive neuroectodermal tumors (PNETs) and Ewing ’s sarcoma (ES) belong to the same family of malignant, small, round cell neoplasms of soft tissue or bone origin. With just around 30 cases reported so far, primary pleuro-pulmonary PNETs/ES are extremely rare in adults. We herein describe a case of a hexagenerian male diagnosed with primary ple uro-pulmonary PNETs/ES after tissue biopsy. He underwent neoadjuvant chemotherapy, and after significant downstaging of the tumor, he underwent complete resection of the lesion through uniportal video-assisted thoracoscopic surgery (u-VATS) followed by adjuvant chemo-ra...

Thrombolytic treatment of right atrial thrombus in a patient with B cell acute lymphoblastic leukemia: a case report
AbstractRight atrial thrombus is a severe complication of central venous catheterization (CVC). Concomitant pulmonary embolism may aggravate the clinical picture by causing or increasing shortness of breath and decreasing effort capacity, palpitations, and tricuspid valve regurgitation. A 32-year-old female patient withB cell acute lymphoblastic leukemia receiving chemotherapy was treated with alteplase thrombolysis because of the development of catheter-related right atrial thrombus and accompanying pulmonary embolism. On echocardiography, it was observed that the thrombus in the right atrium had regressed completely, but...

A novel surgical approach for multiple bronchial artery aneurysms
AbstractBronchial artery aneurysms (BAAs) are rare and are known to be associated with bronchiectasis. The presentation varies from incidental radiological finding to life-threatening hemoptysis. A diagnosis of BAA is an indication for intervention irrespective of its presentation. Despite interventional procedures being at the forefront of management, surgical procedures are being reserved for specific situations. Recently, video-assisted thoracoscopic surgery is an alternate for management of BAA. We, herein, present a case of multiple BAA with cystic bronchiectasis managed surgically with left lower lobectomy and locali...

Predictive value of age, creatinine, and ejection fraction (ACEF) scoring system for operative mortality in patients with Stanford type A aortic dissection
ConclusionsThe ACEF score can be used as a useful and relatively simple tool for risk stratification before TAAD surgery. However, the ACEF score is only indicated for risk assessment and should not affect treatment. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Evaluation of TTK Chitra heart valve prosthesis in pediatric patients
This report highlights the outcome of valve replacement using TTK Chitra heart mechanical valve in a subgroup of pediatric patients This cohort of 27 pediatric patients with implantations during January 2006 to December 2018 was followed up prospectively. The cohort consisted of 12 aortic valve replacement (AVR), 14 mitral valve replacement (MVR), and 1 double valve replacement (DVR) patients. Total follow-up was 254 patient-years (AVR  = 107, MVR = 136, DVR = 11) being 90% complete. The results show that the survival rates and event-free rates were satisfactory. Despite many reservations due to the high risk i...

Left ventricular assist device inflow obstruction
In this report, we present a case of inflow obstruction that was successfully treated with left ventricle myectomy and mitral valvectomy. Transesophageal echocardiogram was essential in diagnosing this condition. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Medium-term outcomes of a modified technique for RSOV-VSD subset
AbstractRupture of sinus of Valsalva (RSOV) aneurysm to right ventricular outflow tract or main pulmonary artery is commonly associated with subaortic ventricular septal defect (VSD). A standard surgical approach described is double patch closure. We have adopted the principle of transaortic direct closure of VSD with prosthetic patch closure of sinus defect; this novel technique is used for past 15  years with good long-term results. We believe this technique appropriately addresses the embryo-pathological basis of RSOV-VSD complex. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

The BEST trial is here — but is anyone listening?
AbstractThe long-term outcome of the Randomized Comparison of Coronary Artery Bypass Surgery and Everolimus-Eluting Stent Implantation in the Treatment of Patients with Multivessel Coronary Artery Disease trial has recently been published. This appraisal provides a critical evaluation of the main findings of the study. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Dual origin of left vertebral artery
We present a case of a 60-year-old female with headache and vertigo where computed tomography (CT) angiography incidentally revealed a dual origin of the left vertebral artery in which the dominant lateral limb of the left vertebral artery was arising from the left subclavian artery while the accessory medial limb was seen arising from the aortic arch with fusion of the two limbs seen cranial to the transverse process of the sixth cervical vertebra. A comparison of mortality and indicators of treatment success of resuscitative endovascular balloon occlusion of aorta (REBOA): a systematic review and meta-analysis
ConclusionsREBOA has a potential for wider application in civilian settings, with our study demonstrating lower in-hospital mortality compared to RT. Prospective multi-centre studies are needed for further evaluation of the indications and feasibility of REBOA.Level of Evidence  + Study Type: Level IV. Systematic review with meta-analysis. Transcatheter aortic valve implantation causing left ventricular perforation may be associated with an aberrant right subclavian artery: a case report
AbstractLeft ventricular perforation is an uncommon complication in transcatheter aortic valve implantation (TAVI). And a case of left ventricular perforation associated with an aberrant right subclavian artery (ARSA) has rarely been reported. An 86-year-old female attended our hospital with symptomatic severe aortic stenosis. Because of the patient ’s advanced age, and that she was a long-term steroid user, we treated her using TAVI. Although the presence of ARSA was noted, the aortic arch was a smooth arc on images of computed tomography (CT) along the axis of the aortic arch. A trans-femoral TAVI was performed. When t...
