Open arch replacement: my way
AbstractWith advancement of hybrid and endovascular techniques, there are very few indications for open arch replacement. Major advancements in open arch replacement include antegrade perfusion-based cerebral protection, and an endovascular compliant arch replacement. In the present article, we demonstrate and describe our technique of Bentall ’s procedure and endovascular compliant arch replacement in a young Marfan’s patient with chronic type A dissection and root aneurysm. Congenital pulmonary airway malformation in a 5  month old boy, complicated by pneumonia
AbstractCongenital cystic lesions of the lung are rare. The most common congenital anomaly among them, involving the lower respiratory tract, is congenital cystic adenomatoid malformation (CCAM), currently referred to as congenital pulmonary airway malformation (CPAM). They may be incidentally detected on prenatal or postnatal imaging. They usually present within two years of life, rarely in adults. CPAMs may be asymptomatic at birth. Some may opt for observation alone due to lack of evidence on the incidence of long-term complications. However, rarely has a CPAM remained asymptomatic throughout life and complications even...

Mitral regurgitation secondary to kissing aortic valve vegetation
AbstractInfective endocarditis is associated with acute heart failure if the valve involvement leads to significant alteration in the hemodynamics. Vegetation in infective endocarditis invades the tissue causing disruption of the anatomical integrity of the valve structure. Heart failure in infective endocarditis is associated with high morbidity and mortality despite advances in surgical and medical therapies. Aortic valve vegetation may cause involvement of mitral valve through different mechanisms. Here, we present a case of infective endocarditis with vegetation on the aortic valve causing perforation of anterior mitra...

Post-thoracotomy intercostal artery pseudoaneurysm manifesting as a chest wall metastasis
We describe the case of a 73-year-old male patient with colonic adenocarcinoma pulmonary metastasis. Repetitive wedge resections and a right lower lobectomy were performed to remove multiple metastatic lesions. At follow-up assessment, the patient reported localized thoracotomy site pain progressing with time and unresponsive to oral analgesics. Chest computed tomography (CT) revealed a pseudoaneurysm of 4-cm diameter of the right 5th intercostal artery. The patient underwent embolization of the lumen and was discharged from the hospital after 24  h. Successive CT re-assessment checks were unremarkable. (Source: Indian Jo...

Posttransplant lymphoproliferative disorder in a heart transplant recipient: a case report
AbstractMalignancy in heart transplant recipients is a grave complication. Post-transplant lymphoproliferative disorder (PTLD) is the second most common tumour in adults and commonest in children. The incidence varies with the transplanted organ from 1 to 2% following kidney transplantation to as high as 10% following thoracic organ transplantation due to different immunosuppression intensity. PTLD include a wide spectrum of diseases ranging from benign proliferation of lymphoid tissue to frank malignancy with aggressive behaviour (lymphoma). Epstein-Barr virus (EBV) infection and prolonged immunosuppressant therapy are im...

Prospective multicenter online testing of the carotid endarterectomy risk stratification calculator carotidscore.ru
ConclusionThe use of CarotidSCORE (carotidscore.ru) allows for the identification of high-risk patients, enabling clinicians to opt for CAS instead of CEA and reduce the incidence of complications. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Colonic transposition using mid colon for corrosive oesophageal strictures
ConclusionA successful reconstruction can be done by securing the correct vascular pedicle and a technique of good anastomosis. From our experience, colonic transposition using mid colon as conduit can be considered as a viable alternative in patients with long segment oesophageal stricture. Off-pump CABG surgery in left main coronary artery disease: a single-center prospective registry
ConclusionsThis study shows that off-pump CABG surgery is safe in patients with LM CAD. There is low in-hospital mortality and MACE and 3-year outcomes are similar to the published data of LM CAD patients who undergo on-pump CABG. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Endobronchial hydatid cyst causing variable intrathoracic airflow limitation selectively during expiration acting as a ball valve
AbstractLung hydatid cyst is a frequently encountered site of echinococcus infection after the liver. Rupture of endobronchial hydatid cyst may lead to hydatemesis or may spread to the normal lung or can cause anaphylactic reactions. Performing bronchoscopy in cases with bronchial hydatid is controversial. The thoracic hydatid cyst may destroy the native lung parenchyma or may cause mass effect on the surrounding structures. Ball valve obstruction of the bronchial hydatid cyst is not documented in the literature and here the authors present such a case as the recognition of the ball valve mechanism is important as it may c...
