EVOLUT Low-Risk Trial data: “We see what we want to see!”
AbstractIn the 4-year update of the EVOLUT Low-Risk Trial (NCT02701283), the authors highlight a non-significant improvement in the primary endpoint and a statistically significant, yet clinically irrelevant, enhancement in hemodynamics with transcatheter aortic valve replacement (TAVR) compared to surgical aortic valve replacement (SAVR). Overlooking the concurrent surgical procedures in SAVR patients, mortality rates remain similar at 4 years. Unaddressed is the substantial increase in permanent pacemaker implantations with TAVR and the fate of patients with paravalvular leaks. Financial burden of coronary artery bypass grafting in India: implications for catastrophic health expenditure and healthcare access
AbstractCardiovascular diseases (CVD) have become a leading cause of mortality in India. High costs of coronary artery bypass grafting (CABG) impose a financial burden in patients often resulting in catastrophic health expenditure (CHE). CHE and distressed financing have significant microeconomic and macroeconomic implications. CHE depletes savings, forces asset disposal, and perpetuates poverty. At a macroeconomic level, the burden of CVD impacts gross domestic product, economic productivity, healthcare budgets, and social welfare. Addressing these challenges requires a multifaceted approach. Prioritizing accessible and a...

Tricuspid valve repair and replacement for infective endocarditis
AbstractInfective endocarditis represents a challenging and life-threatening clinical condition affecting native and prosthetic heart valves, endocardium, and implanted cardiac devices. Right-sided infective endocarditis account for approximately 5 –10% of all infective endocarditis and are often associated with intravenous drug use, intracardiac devices, central venous catheters, and congenital heart disease. The tricuspid valve is involved in 90% of right-side infective endocarditis. The primary treatment of tricuspid valve infective endoc arditis is based on long-term intravenous antibiotics. When surgery is required,...

Dual origin of the left vertebral artery: a rare anatomic variation
We describe a case of a duplicated left vertebral artery. Computed tomography (CT) angiography examination of the head and neck vessels of a 63-year-old man with a history of imbalance was undertaken. A duplicated left vertebral artery was incidentally noted. The medial limb directly originated from the arcus aorta between the left common carotid artery and the left subclavian artery. The lateral limb originated from the proximal left subclavian artery as the first branch and entered the transverse foramen of the C6 vertebra as usual. Prior to neck procedures requiring the anterior cervical approach and carotid endarterect...

Microvascular reconstruction in salvaging the gastric conduit post-esophagectomy after iatrogenic right gastroepiploic vessel injury
In conclusion, immediate microvascular reconstruction offers a viable solution by restoring perfusion of the gastric tube in cases of right gastroepiploic vessel injury during esophagectomy. (Source: Indian Journal of Thoracic and Cardiovascular Surgery)

Successful endovascular management of coral reef aortic occlusion
We present our experience with endovascular management of 10 patients with coral reef aorta. Uncontrolled hypertension, chronic renal disease, disabling claudication, and critical limb ischemia with tissue loss were the presenting symptoms. Seven patients had infrarenal aortic occlusion, and 3 had occlusion at renal and suprarenal aorta. Eight had involvement of the visceral vessels and 3 had renal artery stenosis. Common iliac, femoral and subclavian were the other arteries involved. All procedures were done under local anaesthesia. Aortic stenting was done in 7 and aortoiliac stent in 3. Two had covered stents and the re...

An overview of aortic valve anatomy: the current understanding
This article provides an overview of the changing understanding of aortic root anatomy, shedding light on the intricate structures that contribute to maintaining unidirectional blood flow and the durability of the aortic valve. From historical perspectives to contemporary microscopic details, the components of the aortic root are explored, including the sinutubular junction, aortic sinuses, valve leaflets, and interleaflet triangles. Microscopically, the aortic annulus and leaflets reveal a complex architecture that facilitates blood flow while withstanding lifetime stresses. Additionally, the clinical relevance of aortic ...

Significance and current approaches to vascular graft infection
AbstractVascular graft/endograft infection (VGEI) is a constant in cardiovascular surgery with published rates between 1 and 5%. Every graft type and anatomical location is a potential target for infectious complications. These patients are sick patients with high frailty burden. Management of VGEI entails a multidisciplinary and multimodality approach. Here we review some aspects of the problem of VGEI including prevention, diagnosis, and surgical therapy with focus on recent developments in the field. Stanford type B aortic dissection in a patient with aberrant right subclavian artery
AbstractAn aberrant right subclavian artery is a rare aortic arch anomaly where the right subclavian artery arises from the proximal part of the descending thoracic aorta and distal to origin of left subclavian artery. It usually courses behind the esophagus. Type B aortic dissection along with aberrant right subclavian artery is not common. A middle-aged man presented with complaints of epigastric pain and on evaluation was found to have aberrant right subclavian artery with type B aortic dissection. A total arch replacement with frozen elephant trunk surgery and an extra-anatomic bypass of right subclavian artery were pe...
