More intensive therapy as more effective treatment for frail patients with multiple myeloma
Blood Adv. 2023 Aug 15:bloodadvances.2023011019. doi: 10.1182/bloodadvances.2023011019. Online ahead of print.ABSTRACTAlthough RCT data suggest that the more intensive triplet bortezomib-lenalidomide-dexamethasone (VRd) is superior to the less intensive doublet lenalidomide-dexamethasone (Rd) in patients newly diagnosed with multiple myeloma (MM), guidelines have historically recommended Rd over VRd for frail patients who may not tolerate a triplet. We identified 2573 patients (median age 69.7 years) newly diagnosed with MM who were initiated on VRd (990) or Rd (1583) in the national U.S. Veterans Affairs Healthcare System...

The HPA Axis as Target for Depression
Curr Neuropharmacol. 2023 Aug 11. doi: 10.2174/1570159X21666230811141557. Online ahead of print.ABSTRACTMajor depressive disorder (MDD) is a stress-related mental disorder with a lifetime preva- lence of 20% and, thus, is one of the most prevalent mental health disorders worldwide. Many studies with a large number of patients support the notion that abnormalities of the hypothalamus-pituitary- adrenal (HPA) axis are crucial for the development of MDD. Therefore, a number of strategies and drugs have been investigated to target different components of the HPA axis: 1) corticotrophin- releasing hormone (CRH) 1 receptor antag...

CircFOXN2 alleviates glucocorticoid- and tacrolimus-induced dyslipidemia by reducing FASN mRNA stability by binding to PTBP1 during liver transplantation
CONCLUSION: CircFOXN2 alleviated the dysregulation of lipid metabolism induced by the combination of TAC and Dex by modulating the PTBP1/FASN axis.PMID:37575056 | DOI:10.1152/ajpcell.00462.2022

Crosstalk between glucocorticoid and mineralocorticoid receptors boosts glucocorticoid-induced killing of multiple myeloma cells
Cell Mol Life Sci. 2023 Aug 14;80(9):249. doi: 10.1007/s00018-023-04900-x.ABSTRACTThe glucocorticoid receptor (GR) is a crucial drug target in multiple myeloma as its activation with glucocorticoids effectively triggers myeloma cell death. However, as high-dose glucocorticoids are also associated with deleterious side effects, novel approaches are urgently needed to improve GR action in myeloma. Here, we reveal a functional crosstalk between GR and the mineralocorticoid receptor (MR) that plays a role in improved myeloma cell killing. We show that the GR agonist dexamethasone (Dex) downregulates MR levels in a GR-dependent...

Distinct pulmonary and systemic effects of dexamethasone in severe COVID-19
Res Sq. 2023 Aug 3:rs.3.rs-3168149. doi: 10.21203/rs.3.rs-3168149/v1. Preprint.ABSTRACTDexamethasone is the standard of care for critically ill patients with COVID-19, but the mechanisms by which it decreases mortality and its immunological effects in this setting are not understood. We performed bulk and single-cell RNA sequencing of the lower respiratory tract and blood, and plasma cytokine profiling to study the effect of dexamethasone on systemic and pulmonary immune cells. We find decreased signatures of antigen presentation, T cell recruitment, and viral injury in patients treated with dexamethasone. We identify comp...

Direct drug milling in organic PLGA solution facilitates the encapsulation of nanosized drug into PLGA microparticles
The objective of this study was to prepare poly(lactide-co-glycolide) (PLGA) microparticles loaded with nanosized drug by combining non-aqueous wet bead milling and microencapsulation. 200 - 300 nm dexamethasone, hydrocortisone and dexamethasone sodium phosphate nanosuspensions were successfully prepared by wet bead milling the drug in dichloromethane using PLGA as a stabilizer. PLGA microparticles loaded with nanosized drugs were then prepared by a solid-in-oil-in-water (S/O/W) solvent evaporation method or solid-in-oil-in-oil (S/O/O) organic phase separation method. The microparticles were characterized by laser diffract...
