Homocysteine-lowering interventions for preventing cardiovascular events.
CONCLUSIONS: In this third update of the Cochrane review, there were no differences in effects of homocysteine-lowering interventions in the form of supplements of vitamins B6, B9 or B12 given alone or in combination comparing with placebo on myocardial infarction, death from any cause or adverse events. A Study of Srum Homocysteine Level in Ischaemic Stoke
CONCLUSION: The present study revealed that hyperhomocysteinemia appears to be an important risk factor for ischemic stroke. It is therefore important to use serum homocysteine as an important tool to investigate all cases of ischemic stroke and if serum homocysteine is elevated patient should be put on multivitamins containing vitamin B12, folic acid and pyridoxine to reduce serum homocysteine levels.PMID:35443421

Folic acid can reduce risk of first stroke, according to the results of a double-blind trial. 20 702 people with hypertension without a history of stroke or myocardial infarction enrolled in the China Stroke Primary Prevention Trial were randomly assigned to receive either combined enalapril 10 mg and folic acid 0·8 mg or enalapril 10 mg alone. The trial was terminated early, after a median of 4·5 years, when benefits of the combined treatment were identified: there was a significant reduction in risk of first stroke in the folic acid-enalapril group (hazard ratio 0·79, 95% CI 0·68–0·93) compared with enalapril alone group.

The effect of levodopa benserazide hydrochloride on homocysteinemia levels in patients with Parkinson's disease and treatment of hyperhomocysteinemia.
CONCLUSIONS: Oral administration of Levodopa in the treatment of PD can cause HHcy, which can result in increased occurrence of ischemic stroke. Folic Acid Supplementation and the Risk of Cardiovascular Diseases: A Meta-Analysis of Randomized Controlled Trials Epidemiology
Conclusions Our meta-analysis indicated a 10% lower risk of stroke and a 4% lower risk of overall CVD with folic acid supplementation. A greater benefit for CVD was observed among participants with lower plasma folate levels and without preexisting CVD and in studies with larger decreases in homocysteine levels. Folic acid supplementation had no significant effect on risk of coronary heart disease.

Early Carotid Occlusion After Endarterectomy in a Patient With a Mutation for MTHFR A1298C Polymorphism
Conclusions: Even if the association between homocysteine and carotid restenosis after CEA is unclear, an initial screening before the surgical procedure could be useful to promptly treat this condition and probably reduce the risk of this complication. In the case of homozygous mutation, the selection of patients and the appropriate therapy could reduce the risk of carotid restenosis.
