ALCAM Mediates DC Migration Through Afferent Lymphatics and Promotes Allospecific Immune Reactions
In conclusion, our findings identify ALCAM as a novel therapeutic target for preventing corneal allograft rejection in high-risk patient populations. Moreover, given the documented contribution of ALCAM to (lymph)angiogenesis, DC migration, and T cell activation, ALCAM blockade might represent a therapeutic strategy for treating other immune-mediated inflammatory conditions, such as solid organ transplant rejection, allergy or autoimmune diseases. Experimental Procedures Cloning, Expression, and Purification of I/F8-Fc and KSF-Fc For cloning of I/F8-Fc and KSF-Fc the respective scFv sequence was amplified from previousl...

Downregulation of miR-542-3p Contributes to Apoptosis Resistance in Dermal Fibroblasts from Systemic Sclerosis Patients via Survivin Overexpression.
Abstract Systemic sclerosis (SSc) is characterized by excessive production of collagens by fibroblasts that leads to vast fibrosis. Resistance to apoptosis is one of the possible underlying mechanisms of fibrosis in these patients. Survivinis involved in inhibition of apoptosis and aberrantly functions in SSc. Since dysregulation of survivin-targeting microRNAs (miRNAs) has frequently been observed in cancer and some autoimmune disorders, this study aimed to investigate their expression status in dermal fibroblasts from SSc patients. DiffuseSSc patients were selected according to American College of Rheumatology c...

Sensitivity, specificity, and predictive value of oral mucosal brush biopsy for the diagnosis of peanut allergy
ConclusionIn this pilot study, oral MBB demonstrated high sensitivity and specificity for screening individuals with and without oral cavity clinical reactivity to peanuts and may represent a potentially useful testing method for the diagnosis of peanut allergy in the future.

High Frequency of S2377X mutation in Filaggrin 2 gene among Brazilian Patients with Atopic Dermatitis: no significant association with severity or persistence of disease
We have previously shown that Brazilian patients with moderate-to-severe Atopic Dermatitis (AD) who remained symptomatic despite anti-inflammatory and immunosuppressant therapy presented filaggrin (FLG) expression comparable to control individuals on skin biopsies (JACI 2016;137,AB144). FLG-2 S2377X mutation has been associated with more persistent AD in children of African ancestry. We have investigated FLG-2 mutations in AD patients and correlated results with clinical parameters.

Successful management of type IV hypersensitivity reactions to human insulin analogue with injecting mixtures of biphasic insulin aspart and dexamethasone.
Abstract Although hypersensitivity reaction to insulin was supposed to be less-frequent with current insulin analogue, case reports with different types of allergic reactions to insulin analogue were still reported. The most common form is type I hypersensitivity reaction with IgE-mediated. Besides, type III (IgG and IgM-mediated) and type IV (T-cell mediated delayed reaction) hypersensitivity reactions were also reported. Here we presented a long-standing type 2 diabetes with insulin requirements with hypersensitivity reactions to insulin actrapid, insulin aspart, insulin glargine, insulin detemir, and biphasic i...

Omalizumab in the treatment of eosinophilic granulomatosis with polyangiitis (EGPA): single-center experience in 18 cases
ConclusionsOmalizumab improved asthma control in some patients with EGPA with uncontrolled asthma by reducing asthma exacerbations and oral steroid requirement. However, more data are needed before recommending widespread use of omalizumab in patients with EGPA.

When to Suspect Systemic Mastocytosis?
Conclusions: Most cases of childhood mastocytosis are observed in the form of cutaneous mastocytosis and ıt usually resolves by adolescence. If the patients have cytopenia, hepatosplenomagaly or triptase level >20 ug/l the bone marrow aspiration and biopsy is recommended.DisclosuresNo relevant conflicts of interest to declare.

Drug Allergy.
Abstract Drug allergy is commonly encountered in clinical practice. It is an immunological response to a pharmaceutical agent. The clinical presentation can vary from mild cutaneous reactions to life-threatening conditions such as Stevens-Johnson syndrome and toxic epidermal necrolysis. Diagnosis is most often clinical, but investigations such as measurement of immunoglobulin E, patch testing, and skin biopsy may be required. In patients with a known drug allergy, the offending drug should be avoided. [Pediatr Ann. 2018;47(10):e419-e425.]. PMID: 30308679 [PubMed - in process]

Metal allergy in total-joint arthroplasty: Case report and literature review
This report presents both a case and literature review. Patient concerns: A 61-year-old female patient experienced continuous swelling and pain in the right knee joint for 9 months after a right-side total knee arthroplasty (TKA). Diagnoses: We believe this is the case report of metal allergy in TKA. The following were the reasons for this. First, no definite symptoms of infection during revision arthroplasty were observed, but with obvious hyperplasia of synovium. Furthermore, a frozen biopsy revealed an extremely low neutrophil count, which was considered to be caused by chronic inflammation. Second, the results of...

Anaphylactic Reactions After Discontinuation of Hymenoptera Venom Immunotherapy: A Clonal Mast Cell Disorder Should Be Suspected
ConclusionsLooking at this selected population, we suggest that mastocytosis should be considered in patients developing severe reactions at re-sting after VIT discontinuation and, as a speculation, patients with mastocytosis and HVA should be VIT-treated lifelong.

Mimickers of Urticaria: Urticarial Vasculitis and Autoinflammatory Diseases
Publication date: July–August 2018Source: The Journal of Allergy and Clinical Immunology: In Practice, Volume 6, Issue 4Author(s): Mark D.P. Davis, Jeroen C.H. van der HilstA wide differential diagnosis must be considered in a patient presenting with urticarial plaques. Although acute and chronic urticaria are the commonest diagnoses, other differential diagnoses include polymorphous eruption of pregnancy, mast cell disorders, hypereosinophilic syndrome, urticarial vasculitis, pemphigoid, systemic lupus erythematosus, and autoinflammatory disease. This review will specifically address urticarial vasculitis and autoinflam...
