Increased risk of ischemic stroke in systemic sclerosis: a national cohort study of US veterans.
CONCLUSION: SSc is independently associated with a higher risk of ischemic stroke among U.S. veterans. Patients with SSc represent a population likely to benefit from targeted stroke screening or prevention therapies. PMID: 30877213 [PubMed - as supplied by publisher]

Seize the Day for a Day With No Seizures: Modifiable Midlife Risk Factors Identified.
CONCLUSIONS AND RELEVANCE: Potentially modifiable risk factors in midlife and the APOE ε4 genotype were positively associated with risk of developing late-onset epilepsy. Although stroke and dementia were both associated with late-onset epilepsy, vascular and lifestyle risk factors were significant even in the absence of stroke or dementia. PMID: 30838917 [PubMed]

Use of left atrial appendage occlusion among older cardiac surgery patients with preoperative atrial fibrillation: a national cohort study
ConclusionsSubstantial variability in use of S-LAAO exists. In many instances, the procedure is being deferred in the patients that may be poised to benefit the most (i.e., those with increased CHA2DS2-VASc score-defined stroke risk).

A Nationwide Analysis of Outcomes of Stroke in Hospitalized Patients with Essential Thrombocythemia: 2006 to 2014
Conclusions:Patients with ET who are hospitalized with stroke have significantly worse outcomes. This study demonstrated that a statistically significant difference exists among different age groups of patients with ET and stoke who died during hospitalization when stratification is made using age groups and Charlson Score. This study may serve as an initial point to include new risk factors for further risk stratification. Early identification of patients at higher risk may reduce the incidence and decrease the morbidity of stroke in patients with ET.DisclosuresKota: BMS: Honoraria; Novartis: Honoraria; Xcenda: Honoraria;...

Stroke and Cardiovascular Outcomes in Patients With Carotid Disease Undergoing Transcatheter Aortic Valve Replacement Structural Heart Disease
Conclusions— CD is common among TAVR patients, present in 1 of 5. CD was not associated with an increased risk of stroke or mortality at 30 day or 1 year. Post-TAVR stroke seems to be because of mechanisms other than CD.

New-Onset Cardiovascular Morbidity in Older Adults With Stage I to III Colorectal Cancer.
Conclusion Older patients with colorectal cancer are at increased risk of developing CVD and CHF. Diabetes and hypertension interact with chemotherapy to increase the risk of cardiovascular morbidity. Future studies should assess the potential for personalized therapeutic options for those with preexisting morbidities and for structured monitoring for patients with a history of exposure to chemotherapy regimens, as well as explore the management of preexisting comorbidities to address long-term cardiovascular morbidity. Conclusion: In a cross-sectional study of U.S. adults, we observed some positive associations of uAs and toenail As concentrations with biomarkers potentially relevant to CVD pathogenesis and inflammation, and evidence of a higher capacity to metabolize inorganic As was negatively associated with a marker of oxidative stress. https://doi.org/10.1289/EHP2062 Received: 14 April 2017 Revised: 13 November 2017 Accepted: 15 November 2017 Published: 15 December 2017 Address correspondence to S.F. Hypertension Medication Access and Self-Management Among the Elderly by Education: Insights from the 2013 USA Medicare Current Beneficiary ’s Survey
Hypertension, a major risk-factor for cardiovascular disease and stroke, is a global public health problem. Effective antihypertensive medication use can decrease morbidity/mortality, yet medication access may vary by factors beyond insurance coverage. We measured educational disparities in hypertension medication access and confidence in hypertension self-management among the elderly with Medicare coverage in the USA.

Long-Term Exposure to Road Traffic Noise and Nitrogen Dioxide and Risk of Heart Failure: A Cohort Study
Conclusions: Long-term exposure to NO2 and road traffic noise was associated with higher risk of heart failure, mainly among men, in both single- and two-pollutant models. High exposure to both pollutants was associated with highest risk. https://doi.org/10.1289/EHP1272 Received: 25 October 2016 Revised: 09 August 2017 Accepted: 09 August 2017 Published: 26 September 2017 Address correspondence to M. Abstract P443: Multiple Chronic Conditions in Older Adults: Implications for Clinical Trials & Guidelines in Hypertension Session Title: Hypertension and Aging and Cerebrovascular Disease and Stroke
Conclusions: HTN impacts ~82% of older adults with a higher burden of MCC, and ~75% (27,324/36,533) of Medicare beneficiaries with HTN have a large burden of MCCs. Behavioral health diagnosis, associated with adverse outcomes and costs, are common with MCCs. Clinical care, outcomes and costs for older adults with HTN and MCCs could improve with more representative inclusion in clinical trials and translation through integrated clinical guidelines developed by multi-specialty/disciplinary teams.

Hospital Discharge Disposition of Stroke Patients in Tennessee.
CONCLUSIONS: Our investigation revealed that the hospital discharge disposition pattern of patients with stroke in Tennessee was associated with the key patient characteristics of selected demographics, clinical indicators, and insurance status. These analyses resulted in the development of an easy-to-use predictive tool for early determination of hospital discharge disposition status. PMID: 28863224 [PubMed - in process]
