Adverse Effects of Statins
In Reply Dr Goldstein questions the relationship between cholesterol levels, statins, and hemorrhagic stroke. Evidence for the association between low –total serum cholesterol levels, which is mostly LDL-C, and an increased risk of hemorrhagic stroke has been available since at least the late 1980s. A meta-analysis of 23 studies including 1.4 million participants and 7960 hemorrhagic strokes found that the RR of hemorrhagic stroke was 38% higher (95% CI, 8%-59%) in patients with the highest vs lowest LDL-C values. The Cholesterol Treatment Trialists’ Collaboration meta-analysis of 26 trials showed a nonsignificant incr...

Modern Treatment of Asymptomatic Carotid Stenosis —The Importance of Both Medical Therapy and Carotid Endarterectomy
Each year there are approximately 700  000 new ischemic strokes in the US, and 10% to 15% of them are caused by thromboembolism from a previously asymptomatic carotid stenosis (ACS). Three large randomized clinical trials, published in the 1990s and early 2000s, have demonstrated a significant benefit of carotid endarterectomy (CEA) i n preventing stroke in patients with severe ACS compared with medical therapy alone. They showed a 50% relative risk reduction in 5-year stroke rate and an approximately 1% per year absolute stroke risk reduction. Over the course of the past 2 decades, there has been significant improvement...

Statins in primary prevention of poststroke seizures and epilepsy: A systematic review.
CONCLUSIONS: Data from the literature suggest an association between statin use and a reduced risk of ASPSS after ischemic stroke and a reduced risk of PSE after ischemic and hemorrhagic stroke. Although the certainty of the evidence is low, these findings appear promising and worthy of further investigation. PMID: 32916580 [PubMed - as supplied by publisher]

Statins are 'safe, effective and should be used more widely'
Conclusion This study reviews evidence from randomised controlled trials (RCTs) and observational studies to better evaluate the effects and safety of statin therapy. It provides valuable data on the size of the benefits compared with the risks, informing a topic that has had much media coverage in recent times. The researchers highlight that the benefits of statin therapy for people at risk of cardiovascular disease events far outweigh any possible side effects. But it is still for a doctor and their patient to come to a conclusion about what the best treatment for them may be. If you have been prescribed a statin, it...

Interaction of incidental microbleeds and prior use of antithrombotics with early hemorrhagic transformation: Causative or protective ?
Conclusion: Presence of incidental CMBs was not associated with increased risk for early HT of an ischemic stroke. Interestingly, the prior intake of antiplatelets was found to be protective against HT of ischemic stroke.

Poststroke epilepsy: update and future directions
Stroke is among the most common causes of epilepsy after middle age. Patients with poststroke epilepsy (PSE) differ in several respects from patients with other forms of structural–metabolic epilepsy; not least in age, age-related sensitivity to side effects of antiepileptic drugs (AEDs), and specific drug–drug interaction issues related to secondary-stroke prophylaxis. Encouragingly, there has lately been remarkable activity in the study of PSE. Three developments in PSE research deserve particular focus. First, large prospective trials have established the incidence and risk factors of PSE in the setting of m...

Association between low-density lipoprotein cholesterol and cardiovascular mortality in statin non-users: a prospective cohort study in 14.9 million Korean adults
ConclusionLDL-C had U-curve associations for CVD mortality. The associations and optimal ranges differed across CVD subtypes. Women and older adults had weaker positive associations than men and younger adults. Positive associations with ischaemic heart disease and ischaemic stroke were maintained in adults aged 75 –84 years.

E-107 Carotid artery stenting in patients with symptomatic non-stenotic carotid artery disease - a case series
ConclusionsIn this case series performed over a 10-year period at a high-volume stroke center we show that CAS is safe and efficient in patients with SyNC. Previous studies have established safety and effectiveness of CES in patients presenting with SyNC, however there is a paucity of reports in the literature regarding the effect of CAS in this patient population. Recent guidelines have brought specific attention to high-risk plaque characteristics which suggest that plaque instability plays a major role in the onset of ischemic events, regardless of lumen narrowing. Advanced imaging techniques, such as Optical Coherence ...

Real world adherence to oral anticoagulant in non-valvular atrial fibrillation patients in China.
CONCLUSION: Oral anticoagulation was significantly underused in NVAF patients in China. Age, sex, concurrent drug usage and disease history were associated factors. Improving warfarin adherence was promising to reduce ischemic stroke risk of NVAF patients. PMID: 29022745 [PubMed - as supplied by publisher]

Clinical burden of autosomal dominant polycystic kidney disease.
Abstract There are no specific therapies for autosomal dominant polycystic kidney disease (ADPKD), and clinical data evaluating the effects of non-specific therapies on ADPKD patients are scarce. We therefore evaluated those effects using data from a longitudinal health insurance database collected from 2000-2010. We individually selected patients with and without ADPKD from inpatient data files as well as from the catastrophic illness registry in Taiwan based on 1:5 frequency matching for sex, age, and index year. The hazard ratios (HR) of all-cause mortality, ischemic stroke, hemorrhagic stroke and end-stage ren...

Significant reduction in the LDL cholesterol increases the risk of intracerebral hemorrhage: a systematic review and meta-analysis of 33 randomized controlled trials.
Authors: Cheng Y, Qiao L, Jiang Z, Dong X, Feng H, Gui Q, Lu Y, Liang Y Abstract The dose-dependent pleiotropic effects of statin therapy may have unwanted side effects such as increasing the risk of intracerebral hemorrhage (ICH). The relationships among statin therapy, LDL-cholesterol levels, and ICH risk remain controversial. Here, we conduct a systematic review and meta-analysis of dose-dependent statin therapy and ICH risk. Eligible articles were identified by searching MEDLINE from inception up to December 1, 2018. Reference lists of previous meta-analyses were manually searched to retrieve all relevant publi...

Statins Reduce Bleeding Risk in Patients Taking Oral Anticoagulants for Nonvalvular Atrial Fibrillation: A Retrospective Registry Study
ConclusionsStatins significantly reduced the risk of major bleeding, all-cause mortality, and ischemic events in patients with NVAF taking OACs. Their additive benefits should be considered in routine practice and thus be further researched.

Statin Use and Brain Hemorrhage Real Risk or Unfounded Fear?
The relationship between statin use and intracerebral hemorrhage (ICH) has not been definitively established. Studies show both benefit and detriment. Statins may decrease the level of platelet aggregation and thrombogenesis and thus worsen an ICH, but they also modulate the immune system, inhibit the inflammatory process, and improve cerebral blood flow, promoting neuroprotection and tissue recovery. Despite physiological and clinical evidence on both sides of the argument, the idea that statins should be avoided whenever brain hemorrhage is involved has permeated stroke practice. Indeed, early and more recent epidemiolog...

Risk of Intracranial Hemorrhage from Statin Use in Asians: A Nationwide Cohort Study.
CONCLUSIONS: -Generally, no association was observed between cumulative statin use and risk of ICH among subjects without a prior history of stroke. An increased risk was identified among the non-hypertensive cohort, but this finding should be interpreted with caution. PMID: 25858194 [PubMed - as supplied by publisher]
