Women who decline blood during labour: Review of findings and lessons learnt from 52  years of Confidential Enquiries into maternal mortality in the United Kingdom (1962-2019)
CONCLUSIONS: Guidelines from the UK's Royal College of Obstetricians and Gynaecologists and Royal College of Surgeons highlight the need for collaborative, Montgomery-competent discussions during the antenatal period, as well as the engagement of local Jehovah's Witness Hospital Liaison committees. Consultant-led care, antenatal optimisation of haemoglobin and techniques to mitigate blood loss at delivery are paramount. We advocate using a lower threshold for hysterectomy than was used in the cases analysed, for example when the haemoglobin level drops below 8-9 g/l in the context of ongoing bleeding. As patients increasin...

Treatment Options After a Diagnosis of Early Miscarriage: Expectant, Medical, and Surgical
CONCLUSION: If there is no acute indication for the surgical treatment of an early miscarriage, the patient can choose among three treatment options. Expectant and medical treatment can be provided on an outpatient basis. Curettage is the treatment of choice in the presence of infection, marked and persistent bleeding, hemodynamic instability, or a pre-existing coagulopathy.PMID:34696822 | DOI:10.3238/arztebl.m2021.0346 (Source: Deutsches Arzteblatt International)

Risk-Reducing Salpingo-Oophorectomy and the Use of Hormone Replacement Therapy Below the Age of Natural Menopause: Scientific Impact Paper No. 66
BJOG. 2021 Oct 20. doi: 10.1111/1471-0528.16896. Online ahead of print.ABSTRACTThis paper deals with the use of hormone replacement therapy (HRT) after the removal of fallopian tubes and ovaries to prevent ovarian cancer in premenopausal high risk women. Some women have an alteration in their genetic code, which makes them more likely to develop ovarian cancer. Two well-known genes which can carry an alteration are the BRCA1 and BRCA2 genes. Examples of other genes associated with an increased risk of ovarian cancer include RAD51C, RAD51D, BRIP1, PALB2 and Lynch syndrome genes. Women with a strong family history of ovarian...

10.N. Workshop: Women's health reports in Europe - From data to action
Abstract The great impact of gender on health is well researched and documented. In addition to the different morbidity rates of women and men, there are differences in the perception and communication of symptoms, in health-related behaviour and in the use of care services. The causes are primarily social and societal in nature. They are closely linked to biological gender differences. A close look at women's health is not only important to achieve improvements in health and well-being for 51% of the population. Evidence shows that healthy women are the cornerstone of healthy societies. In recent decades, women's health...

Maternal Sepsis: A Review of National and International Guidelines
Conclusion In conclusion, national and international maternal sepsis guidelines were incongruent with each other and SEPSIS-3 in diagnostic criteria and tools but similar in evaluation and management recommendations. An audit of the effect of case selection on compliance with a 30-minute audit standard for decision-to-delivery interval at category 1 caesarean section
The joint Royal College of Obstetricians and Gynaecologists / Royal College of Anaesthetists (RCOG/RCoA) Good Practice Statement on classification of urgency of caesarean section1 states that “a target Decision-Delivery Interval (DDI) for caesarean section for ‘fetal compromise’ of 30 minutesis an audit tool that allows testing of the efficiency of the whole delivery team and has become accepted practice…”.The Lucas classification of urgency used in the UK defines category 1 as ‘Maternal or fetal compromise – immediate threat to life of woman or fetus’ and category 2 as ‘Maternal or fetal compromise – n...

P.90 Impact of coagulation result delays on transfusion practice in major obstetric haemorrhage
Introduction: The 2016-18 MBRRACE report commented that in some women who died from major obstetric haemorrhage (MOH), there was a delay in blood product administration due to delays in reporting coagulation tests.1 The Royal College of Obstetricians and Gynaecologists (RCOG) recommend empirical transfusion of blood products where haemorrhage is ongoing and coagulation results are unknown.2 Our primary objective was to examine the average length of time for coagulation studies to be processed with a result available on our electronic database and assess the association between coagulation results and products given.
