Medical Treatment of COPD.
CONCLUSION: The observed deviations from the recommended treatment, some of which were substantial, might lead to suboptimal treatment outcomes as well as to avoidable side effects of medication. PMID: 30282573 [PubMed - in process] (Source: Deutsches Arzteblatt International)

The Diagnosis and Treatment of Anxiety Disorders.
CONCLUSION: In general, anxiety disorders can now be effectively treated. Patients should be informed of the therapeutic options and should be involved in treatment planning. Current research efforts are centered on individualized and therefore, it is hoped, even more effective treatment approaches than are available at present. Cancers Due to Smoking and High Alcohol Consumption.
CONCLUSIONS: A considerable proportion of cancer cases are attributable to smoking and high alcohol consumption. The Effect of Ambulance Response Time on Survival Following Out-of-Hospital Cardiac Arrest.
Authors: Bürger A, Wnent J, Bohn A, Jantzen T, Brenner S, Lefering R, Seewald S, Gräsner JT, Fischer M Abstract BACKGROUND: Out of hospital cardiac arrest (OHCA) is one of the more common causes of death in Germany. Ambulance response time is an important planning parameter for emergency medical services (EMS) systems. We studied the effect of ambulance response time on survival after resuscitation from OHCA. METHODS: We analyzed data from the German Resuscitation Registry for the years 2010-2016. First, we used a multivariate logistic regression analysis to determine the effect of ambulance response ...

Depressive Symptoms in Adolescents.
In this study, we determined the current prevalence of depressive symptoms in adolescents in Germany. METHODS: A sample of 1001 adolescents aged 12 to 17 that was representative for Germany was surveyed in August and September 2017 through telephone interviews about depressive symptoms in the two weeks leading up to the interview and about the subjects' psychosocial features. The instrument that was used, called DesTeen, includes questions about depressed mood, loss of interest, loss of energy, feelings of worthlessness, guilt feelings, and cognitive symptoms. RESULTS: Prevalences could be estimated and associated ...

Arterial Hypertension.
Authors: Jordan J, Kurschat C, Reuter H Abstract BACKGROUND: Essential arterial hypertension is one of the main treatable cardiovascular risk factors. In Germany, approximately 13% of women and 18% of men have uncontrolled high blood pressure (≥ 140/90 mmHg). METHODS: This review is based on pertinent publications retrieved by a selective literature search in PubMed. RESULTS: Arterial hypertension is diagnosed when repeated measurements in a doctor's office yield values of 140/90 mmHg or higher. The diagnosis should be confirmed by 24-hour ambulatory blood pressure monitoring or by home measurement. Furt...

Endoscopic or Surgical Resection for Gastro-Esophageal Cancer.
CONCLUSION: Borderline cases between endoscopic and surgical resection of early carcinoma of the esophagus or stomach must be managed with an interdisciplinary treatment algorithm. If there is a risk of lymph node metastasis, surgical oncological resection is indicated. Such resections of gastroesophageal cancer in the locally advanced stage should always be part of a multimodal treatment approach. Differences in Breast Cancer Characteristics by Mammography Screening Participation or Non-Participation.
CONCLUSION: MSP participants with invasive breast cancer can generally be treated with less intensive surgical and systemic therapy than non-participants, even if interval cancers are also taken into account. The Impact of Cancer Screening on All-Cause Mortality.
CONCLUSION: Because the proportion of cancer deaths in all deaths in Western Europe is relatively low, cancer screening procedures can reduce all-cause mortality by only 1-3%. Therapeutic Treatment for Abdominal Obesity in Adults.
CONCLUSION: Overall, the identified interventions showed a moderate effect on WC. One reason may be that in most studies WC was a secondary outcome parameter, while only a small number of interventions primarily targeted abdominal obesity. Further research regarding the treatment of abdominal obesity is urgently needed. PMID: 30135008 [PubMed - in process] (Source: Deutsches Arzteblatt International)

Antibiotic-Resistant E. coli in Uncomplicated Community-Acquired Urinary Tract Infection.
CONCLUSION: E. coli with resistance to TMP was significantly less prevalent among the study patients with uncomplicated UTIs than in the routine data of the ARS. Accordingly, TMP should still be considered as an option for the treatment of uncomplicated UTIs. TMP/SMX is considered the agent of second choice because of its side effects. Surveillance systems based on routine data do not yield a representative sample for the evaluation of the resistance situation in patients with uncomplicated UTIs. Drug Hypersensitivity.
CONCLUSION: Careful, structured diagnostics in case of suspected hypersensitivity together with adequate documentation (allergy passport) is necessary in order to avoid incidents in patients receiving subsequent treatment. CONCLUSION: Hypoxemia should certainly be avoided, but the fact that the liberal administration of oxygen to patients in intensive care units and emergency rooms tends to increase morbidity and mortality implies the advisability of a conservative, normoxic oxygenation strategy.
