IJERPH, Vol. 20, Pages 4214: Piloting a Nurse-Led Critical Care Outreach Service to Pre-Empt Medical Emergency Team Calls and Facilitate Staff Learning
knall A nurse-led critical care outreach service (NLCCOS) can support staff education and decision making in the wards, managing at-risk patients with ward nurses to avoid further deterioration. We aimed to investigate the characteristics of patients identified as at-risk, the types of treatments they required to prevent deterioration, the education initiated by the NLCCOS, and the perceived experiences of ward nurses. This prospective observational pilot study using mixed methods took place in one medical and one surgical ward at a university hospital in Denmark. Participants were patients nominated as at-risk by head...

Standardized airway wall thickness Pi10 from routine CT scans of COPD patients as imaging biomarker for disease severity, lung function decline, and mortality
CONCLUSION: In summary, Pi10 was found to be predictive for the course of the disease in COPD, in particular mortality. The fact that Pi10 was lower in patients with severe COPD receiving triple therapy might hint toward additional effects of this functional therapy on airway remodeling.REGISTRATION: ClinicalTrials.gov, Identifier: NCT01245933.PMID:36718763 | DOI:10.1177/17534666221148663 (Source: Respiratory Care)

Patients' Experiences With Therapeutic Approaches for Post-COVID Syndrome: Results of a Crowdsourced Research Survey
Ann Fam Med. 2023 Jan-Feb;21(1):73-75. doi: 10.1370/afm.2898.ABSTRACTSome patients develop multiple protracted sequelae after infection with SARS-CoV-2, collectively known as post-COVID syndrome or long COVID. To date, there is no evidence showing benefit of specific therapies for this condition, and patients likely resort to self-initiated therapies. We aimed to obtain information about therapies used by and needs of this population via inductive crowdsourcing research. Patients completed an online questionnaire about their symptoms and experiences with therapeutic approaches. Responses of 499 participants suggested few a...

High-proportion breast milk feeding is associated with a reduction in the incidence of IVH in very preterm infants
ConclusionHigh-proportion breast milk feeding and breast milk feeding more than 50% of total intake during the 1st week might be protective factors for IVH grade I–II in VPIs, which further verified the neuroprotective effect of breast milk. In clinical practice, the construction of breast milk banks should be strengthened, breast milk feeding should be encouraged in neonatal intensive care units, and efforts should be made to increase breast milk feeding rates to improve the outcomes of VPIs. Characterization of SARS-CoV-2 Aerosols Dispersed During Noninvasive Respiratory Support of Patients With COVID-19
CONCLUSIONS: Our findings demonstrate very limited detection of SARS-CoV-2-containing aerosols in the vicinity of subjects with COVID-19 receiving NRS therapies in the clinical setting. These results, combined with controlled chamber measurements showing that HFNC and NIV device usage was not associated with increased aerosol dispersion, suggest that NRS therapies do not result in increased dispersal of aerosols in the clinical setting.PMID:36566031 | DOI:10.4187/respcare.10340 (Source: Respiratory Care)

Usefulness of the early combination of non-invasive respiratory therapies in patients with COVID-19
Conclusions: The combination of HFNC+CPAP, especially in the first 24 hours after IRCU admission, is a useful tool in the management of SARS-CoV-2 pneumonia.BaselineAll patients (n=760)EHC (n=380)LHC (n=380)p valueAge (SD)56.5 (12.9)54.6 (12.9)58.4 (12.6)0.001Men (%)502 (66.1)248 (65.3)254 (66.8)0.64Charlson Comorbidity Index (SD)1.99 (1.87)1.78 (1.81)2.19 (1.9)0.008Obesity (%)356 (46.8)210 (55.3)146 (38.4)0.001ETIORCI 95%p value30-day mortalityORCI 95%p valueAge1.071.02-1.110.001Age1.081.04-1.110.001Charlson Comorbidity Index0.980.77-1.240.89Charlson Comorbidity Index1.211.02-1.420.02EHC0.480.26-0.860.01EHC0.550.33-0...

Underrepresentation of Respiratory Therapists as Experts in Delphi Studies on Respiratory Practices and Research Priorities
Respir Care. 2022 Dec;67(12):1609-1632. doi: 10.4187/respcare.10012.ABSTRACTDelphi survey techniques are a common consensus method used to collect feedback from an expert panel to inform practices, establish guidelines, and identify research priorities. Collecting respiratory therapists' (RT) expertise and experiences as part of consensus-building methodologies is one way to ensure that they align with RT practices and to better influence respiratory care practice. This narrative review aimed to report the RT representation in expert panels of Delphi studies focused on respiratory therapy practices and research priorities....

Hospital-level care at home for patients with acute respiratory disease: A descriptive analysis
Chest. 2022 Nov 10:S0012-3692(22)04067-3. doi: 10.1016/j.chest.2022.11.006. Online ahead of print.ABSTRACTBACKGROUND: Home hospital (HH) is hospital-level substitutive care delivered at home for acutely ill patients who would traditionally be cared for in the hospital. Despite HH programs operating successfully for years, and scientific evidence of similar or better outcomes compared to bricks and mortar care, HH outcomes in the US for respiratory disease have not been evaluated.RESEARCH QUESTION: Do outcomes differ between patients admitted to HH with acute respiratory illness vs other acute general medical conditions?STU...

Validation of a structured questionnaire to assess the perception and satisfaction of respiratory therapy students toward career prospects and learning resources
CONCLUSION: This study shows a clinically acceptable model, it fits and suggests the possibility of applying a SQ to a respiratory therapy student with relatively good construct validity and internal consistency, based on the results of CFA.PMID:36299617 | PMC:PMC9541297 | DOI:10.29390/cjrt-2022-032 (Source: Respiratory Care)
