Glaucoma: Diagnosis and Management
Am Fam Physician. 2023 Mar;107(3):253-262.ABSTRACTGlaucoma is a group of eye disorders characterized by progressive deterioration of the optic nerve that can lead to vision loss. Primary open-angle glaucoma (POAG) is the most common form in the United States. The risk of POAG increases with age, family history of glaucoma, type 2 diabetes mellitus, hypotension, hypothyroidism, obstructive sleep apnea, cardiovascular disease, and myopia. Up to one-half of patients are undiagnosed because a diagnosis often requires monitoring over years to document changes suggesting POAG. These include a cup-to-disc ratio of 0.3 or greater,...

Argon laser trabeculoplasty for congenital ectropion uveae: Case report and review of the literature
Eur J Ophthalmol. 2023 Feb 7:11206721231156501. doi: 10.1177/11206721231156501. Online ahead of print.ABSTRACTA 18-year-old woman with unilateral glaucoma secondary to congenital ectropion uveae (CEU) in the right eye, without other systemic and ocular associations. Intraocular pressure (IOP) was uncontrolled under topical therapy. The visual field (VF) was within normal limits; retinal nerve fiber layer (RNFL) thickness, as evaluated by Optical Coherence Tomography (OCT), was reduced. The patient refused surgical intervention. After evaluation of the iridocorneal angle, ALT was performed as follows: 95 spots were applied ...

Racial Disparities Affecting Black Patients in Glaucoma Diagnosis and Management
Semin Ophthalmol. 2023 Jan 20:1-11. doi: 10.1080/08820538.2023.2168489. Online ahead of print.ABSTRACTBlack patients are more affected by glaucoma and suffer from more advanced disease. Diagnostic challenges among black patients with glaucoma include lower rates of diagnostic testing and thinner average central corneal thickness, the latter of which affects intraocular pressure measurement. Treatment challenges include poor follow-up, medication adherence, and trust in providers. Black patients undergoing trabeculectomy have higher rates of failure compared to white patients. Race is not a definitive factor affecting succe...

Differentiating stages of functional vision loss from glaucoma using the Disc Damage Likelihood Scale and cup:disc ratio
CONCLUSION: This study indicates that both structural staging systems can differentiate severe/end-stage glaucoma from less severe disease, with a moderate advantage of DDLS over CDR. Clinical examination of the optic disc plays an important role in addition to functional assessment when managing severe/end-stage glaucoma.PMID:36653163 | DOI:10.1136/bjo-2022-321643 (Source: The British Journal of Ophthalmology)

Effectiveness and safety of VISULAS ® green selective laser trabeculoplasty: a prospective, interventional multicenter clinical investigation
ConclusionsSLT performed with the VISULAS® green laser achieved clinically significant additional IOP reductions in medically treated as well as in treatment na ïve eyes with POAG and there were no relevant safety issues. The results are comparable to other reported SLT studies. (Source: International Ophthalmology)

Direct selective laser trabeculoplasty in open angle glaucoma study design: a multicentre, randomised, controlled, investigator-masked trial (GLAUrious)
Introduction Laser trabeculoplasty is an effective and widely used treatment for glaucoma. A new laser technology, the Eagle direct selective laser trabeculoplasty (DSLT) device, may provide automated, fast, simple, safe and effective laser treatment for glaucoma in a broader range of clinical settings. This trial aims to test the hypothesis that translimbal DSLT is effective and not inferior to selective laser trabeculoplasty (SLT) in reducing intraocular pressure (IOP) in open angle glaucoma (OAG). Methods and analysis This is a multicentre, randomised, controlled, investigator-masked study. The primary efficacy outcome...
