Real-time EndoFaster improves Helicobacter pylori detection in chronic active gastritis
Helicobacter pylori is the most frequent cause of chronic active gastritis (CAG), namely the first step for gastric cancer development. When infection is not detected at histology, another test is advised. EndoFaster is novel device that reveal the presence of H. pylori by determining ammonium concentration in the gastric juice during endoscopy. We evaluated whether this test may improve etiological diagnosis in CAG patients. In 595 consecutive patients who underwent upper endoscopy gastric juice was analysed with EndoFaster and standard biopsies were taken. CAG with typical bacteria was detected in 102 (17.1%) patients, a...

Kinetics of SARS-CoV-2 antibody responses pre-COVID-19 and post-COVID-19 convalescent plasma transfusion in patients with severe respiratory failure: an observational case-control study
Conclusions CCP transfusion did not increase the kinetics of SARS-CoV2 antibodies and did not result in improved clinical outcomes in patients with COVID-19 with severe respiratory failure, suggesting that CCP may not be indicated in this category of patients. Correlation of BRAF mutational status with clinical characteristics and survival outcomes of patients with ameloblastoma: the experience of 11 Italian centres
Conclusions Local recurrence occurs in 30% of patients with ameloblastoma. BRAFV600 mutation is associated with younger age, mandibular localisation and with unicystic ameloblastoma. Neither BRAF mutation nor microscopically positive surgical margins were associated with RFI. Further studies are needed to elucidate outcomes of this rare disease according to clinical, histopathological and comprehensive molecular features. (Source: Journal of Clinical Pathology)

Improving the management of early colorectal cancers (eCRC) by using quantitative markers to predict lymph node involvement and thus the need for major resection of pT1 cancers
Conclusions In this study, we were able to reduce the potential resection rate of pT1 with the multivariate qualitative and/or quantitative model to 34% or 35% while detecting 95% or 90% of all LNM cases, respectively. While these findings need to be validated, this model could lead to a reduction of the major resection rate in eCRC. (Source: Journal of Clinical Pathology)

Update on the diagnosis and management of the autosomal dominant acute hepatic porphyrias
The autosomal dominant acute hepatic porphyrias (AHPs), acute intermittent porphyria, hereditary coproporphyria (HCP) and variegate porphyria (VP), are low penetrance adult onset disorders caused by partial deficiency of enzymes of haem biosynthesis. All are associated with acute neurovisceral attacks, which are a consequence of the increased hepatic demand for haem triggered by hormones, stress, drugs or systemic infections which leads to upregulation of the pathway and overproduction of haem precursors 5-aminolaevulinic acid (ALA) and porphobilinogen (PBG). Acute episodes are characterised by severe abdominal pain, nause...

Clinical utility of pathology data: endometrial and tubo-ovarian carcinomas
Cancer resection specimens are usually reported using standardised proformas that consist of a list of elements, which include core (required) and non-core (recommended) items. Although all elements are generally included in the reports, the clinical importance of a particular parameter often depends on a variety of factors, including the clinical setting, local management guidelines and other pathological parameters. In this review, we briefly outline how histopathology data are used to guide management of patients with endometrial and tubo-ovarian cancers, the most common gynaecological malignancies, and provide advice a...

Histopathology reporting for personalised medicine: focus on clinical utility
Histopathology guidelines generally focus on standardised collection of data items to facilitate completeness and reproducibility of histopathology reporting. A data item is categorised as either core (mandatory) or non-core (recommended but not mandatory), irrespective of the clinical scenario. However, a data item that is critical for patient management in one clinical setting may have little clinical significance in another setting. A diagnosis of limited extent Gleason score 3+3=6 prostate cancer is critical in a patient being investigated for raised serum prostate-specific antigen but would be clinically irrelevant in...

Clinical utility of GI pathology data: implications for practising pathologists
We describe the potential clinical relevance of selected pathology data items pertinent to specific conditions and highlight areas of contention with respect to the significance of some pathology features. Recent guidelines are described where a change, for example, in diagnostic criteria for a condition is described, or criteria influencing further management such as endoscopic surveillance. The aim of this review is to focus on the clinical importance of careful written communication between the pathologist and primary clinician, illustrated by selective clinical scenarios involving the upper and lower GI tracts. (Source...

Clinical utility of breast pathology data: implications for practising pathologists
In breast cancer, the quality of the pathology services is of paramount importance as inevitably, the pathologist makes the confirmatory diagnosis and provides prognostic and predictive information, informing treatment plans directly. Various national and international organisations provide a pathology reporting minimum dataset (MDS) to ensure consistency in reporting. While the use of MDS promotes clarity, there may be specific areas requiring the pathologist’s input for individual patients and hence pathologists need to be aware of the clinical utility of pathology data to help tailor individualised patient treatme...

Clinical utility of histopathology data: urological cancers
Cancer datasets recommend standardised reporting of histopathological data items with elements categorised as either core (required) or non-core (recommended), irrespective of the clinical scenario. However, the clinical significance of a data item in an individual case would depend on the clinicopathological setting as well as local management guidelines. A data item that is critical for patient management in one clinical scenario may be largely irrelevant in another patient. Pathologists must understand how their data are used in clinical practice so that they can focus their limited resources appropriately. We briefly r...

Clinical utility of pathology data
The practice of pathology is constantly evolving, and much of what we do is data-driven. Precision medicine requires precision pathology. This encompasses a wide remit of modalities that have diagnostic, therapeutic and prognostic ramifications. This has necessitated a greater depth and scope of reporting by pathologists. The resultant information deluge has been organised into checklists that attempt to achieve a similar standard across countries and organisations. These guideline templates are being updated and refined to incorporate the ever-lengthening list of parameters and information that is generated. Byproducts of...

Letter to the editor: additional evidence to support a cross-reactivity of SARS-CoV-2 with HIV chemiluminescent assay
During the COVID-19 pandemic, a 43-year-old male patient with no clinically relevant changes, underwent routine examinations at the Brazilian Army Institute of Biology on 25 May 2021. The fourth-generation chemiluminescence assay, Vitros HIV Combo (Ortho Clinical Diagnostics, New Jersey, USA), for HIV-1 and HIV-2 was performed. The Vitros HIV Combo test detects both HIV-1 and HIV-2 antibodies and the p24 antigen in acute cases of HIV infection. The result obtained was 2.54 s/CO (sample relative light unit/cut-off relative light unit). Specimens with s/CO values of ≥1 are considered reactive for HIV. Following the flowch...
