Top tips on the management of antithrombotic agents in the periendoscopic period
One dilemma that all endoscopists face on a daily basis is how to manage antithrombotic agents in the setting of interventional procedures. Much data are available in the literature on this topic, and it can be confusing and overwhelming to readers to try to parse through it all. The interventions we do are of highly variable risk, and patients can be on any of a wide array of agents, further complicating matters. In this Top Tips, Dr Alberto Tringali reviews his experience and knowledge in this matter and provides a plethora of information on how to manage patients on antithrombotic agents who need endoscopy, taking into ...

Clofazimine-associated enteropathy
A 36-year-old man presented to the gastroenterology department because he had experienced intermittent abdominal pain for 1 year. He had received a diagnosis of multidrug-resistant pulmonary tuberculosis 2 years earlier and had received a drug regimen containing clofazimine, linezolid, protionamide, cycloserine, pyrazinamide, and moxifloxacin for 19 months. The abdominal pain occurred after he had been taking clofazimine for 7 months. Physical examination showed skin darkening. Laboratory results showed no abnormalities. Insulin sensitivity and beta cell function after Duodenal Mucosal Resurfacing (DMR): An Open-Label, Mechanistic, Pilot Study
The duodenum has been shown to play a key role in glucose homeostasis. Duodenal mucosal resurfacing (DMR) is an endoscopic procedure for patients with type 2 diabetes (T2D) in which the duodenal mucosa is hydrothermally ablated. DMR improves glycemic control, but the underlying mechanisms remain unclear. Here, we report changes in glucoregulatory hormones and indices of insulin sensitivity and beta cell function after DMR. Incomplete Mucosal Layer Excision during Endoscopic Mucosal Resection: a potential source of recurrent adenoma
Residual or recurrent adenoma detected during surveillance (RRA) is the major limitation of endoscopic mucosal resection (EMR). The pathogenesis of RRA is unknown although thermal ablation of the post-EMR defect (PED) margin reduces RRA. We aimed to identify a feature within the PED which could be associated with RRA. Safety of endoscopic ultrasonography latex balloon use in patients with a latex allergy
Balloons are used in endoscopic ultrasonography (EUS) to improve visualization. However, data on the safety of latex balloons in patients with latex allergies is limited, and non-latex alternatives can be costly. We aimed to investigate the safety of latex balloon use during EUS. (Source: Gastrointestinal Endoscopy)

Impact of study design on adenoma detection in the evaluation of artificial intelligence –aided colonoscopy: a systematic review and meta-analysis
Randomized controlled trials (RCTs) have reported that artificial intelligence (AI) improves endoscopic polyp detection. Different methodologies —namely, parallel and tandem designs—have been used to evaluate the efficacy of AI-assisted colonoscopy in RCTs. Systematic reviews and meta-analyses have reported a pooled effect that includes both study designs. However, it is unclear whether there are inconsistencies in the reported results o f these 2 designs. Here, we aimed to determine whether study characteristics moderate between-trial differences in outcomes when evaluating the effectiveness of AI-assisted polyp detec...

Role of the industry representative in the practice of gastroenterology and GI endoscopy
The practice of gastroenterology and GI endoscopy has evolved over the last several decades with numerous scientific and technologic advances in medical therapies, procedural devices, and techniques. Optimal patient care requires clinicians to adopt these new therapeutic tools as evidence-based medicine demonstrates improvement in clinical and patient reported outcomes. For GI endoscopy, providers and staff engage in delivering endoscopic services ranging from basic general endoscopic procedures in an ambulatory surgical center to advanced and more complex interventions performed in inpatient endoscopy facilities. Clinical outcomes of argon plasma coagulation for the treatment of gastric low grade dysplasia
Argon plasma coagulation (APC) could be considered a treatment modality for small gastric low grade dysplasia (LGD) instead of endoscopic resection (ER). Our study investigated the clinical outcomes of APC for treating gastric LGD and associated variables with local recurrence. (Source: Gastrointestinal Endoscopy)

Endoscopic Gastrointestinal Placement of Capsule Endoscopy to Investigate the Small Bowel: a Multicentric European Retrospective Series of 630 Procedures in Adult Patients
Small bowel (SB) capsule endoscopy (CE) is a first line procedure for exploring the SB. Endoscopic GastroIntestinal PlacemenT (EGIPT) of SB CE is sometimes necessary. While the experience of EGIPT is large in pediatric populations, we aimed to describe the safety, efficacy and outcomes of EGIPT of SB CE in adult patients. (Source: Gastrointestinal Endoscopy)
