We read with great interest an article by Youssef et  al1 reporting their investigation of risk factors for pancreatic cancer incidence among patients with intraductal papillary mucinous neoplasms (IPMNs). Oblique fiber preservation in the prevention of GERD after peroral endoscopic myotomy
We read with great interest the article entitled "Conventional versus oblique fiber-sparing (OFS) endoscopic myotomy for achalasia cardia: a randomized controlled trial."1 This single-center randomized controlled trial from India compared conventional myotomy (CM) with OFS myotomy in peroral endoscopic myotomy (POEM) and found that the incidence of post-POEM GERD was similar in both groups. We appreciate the interest of Fujiyoshi and Teshima1 in our randomized trial2 comparing conventional myotomy (CM) and oblique fiber-sparing myotomy (OFS) for achalasia cardia. The incidence of significant reflux esophagitis ( ≥LA B) was not statistically different between the 2 groups. We express our gratitude to Estevinho et  al1 for their insightful comments and inquiries regarding our case report,2 which was recently published in Gastrointestinal Endoscopy. We have adapted this technique for the closure of large mucosal defects after endoscopic submucosal dissection, achieving excellent results in terms of adverse ev ent prevention.3 Unlike the purse-string technique, which typically involves only 1 endoloop, we use multiple endoloops and clips, allowing for the closure of ulcers>20  mm. We read with great appreciation the insightful comments by Oyama et  al.1 They state that patients with intraductal papillary mucinous neoplasms (IPMNs) are at risk for the development of IPMN-derived carcinoma and are also at an elevated risk for the development of concomitant pancreatic ductal adenocarcinoma (PDAC). In their longitudinal cohort of 1404 patients w ith branch-duct IPMN, 68 patients experienced pancreatic carcinoma: 38 (55.9%) had IPMN-derived carcinoma, and 30 (44.1%) had concomitant PDAC.
