We read with great appreciation the insightful comments by Oyama et  al.1 They state that patients with intraductal papillary mucinous neoplasms (IPMNs) are at risk for the development of IPMN-derived carcinoma and are also at an elevated risk for the development of concomitant pancreatic ductal adenocarcinoma (PDAC). In their longitudinal cohort of 1404 patients w ith branch-duct IPMN, 68 patients experienced pancreatic carcinoma: 38 (55.9%) had IPMN-derived carcinoma, and 30 (44.1%) had concomitant PDAC. Endoscopic Submucosal Dissection for Early Esophageal Adenocarcinoma: Low Rates of Metastases in Mucosal Cancers with Poor Differentiation
