Endoscopic ultrasound –guided cholecystojejunostomy for management of acute cholecystitis in a patient with cirrhosis, ascites, and Roux-en-Y gastric bypass anatomy
A 43-year-old woman with decompensated cirrhosis, alcohol use disorder, Roux-en-Y gastric bypass for obesity, and chronic pancreatitis presented with acute cholecystitis (A). The patient was deemed a nonsurgical candidate because of her multiple comorbidities, and percutaneous drainage was not feasible because of significant ascites. Thus, EUS-guided gallbladder drainage was pursued. (Source: Gastrointestinal Endoscopy)

Endoscopist adenomas-per-colonoscopy detection rates and risk for postcolonoscopy colorectal cancer: Data from the New Hampshire Colonoscopy Registry
Adenomas per colonoscopy (APC) may be a better measure of colonoscopy quality than adenoma detection rate (ADR) because it credits endoscopists for each detected adenoma. There are few data examining the association between APC and postcolonoscopy colorectal cancer (PCCRC) incidence. We used data from the New Hampshire Colonoscopy Registry to examine APC and PCCRC risk. (Source: Gastrointestinal Endoscopy)

Continuous wound irrigation and drainage for treating large contained duodenal perforation after endoscopic mucosal resection
A 59-year-old woman experienced a contained duodenal perforation 3 days after undergoing EMR for a 50  × 15 mm duodenal laterally spreading duodenal tumor. Endoscopy confirmed the duodenum contained perforation with abscess and sinus tract formation (A). The patient refused surgery; thus, we decided to treat the condition endoscopically. Because the postoperative wound was large, accounting for a bout three-quarters of the duodenal lumen, and the wound still showed the presence of infection after flushing, simple closure (including over-the-scope clip closure) would have been challenging and not conducive to wound heali...

Endoscopic ultrasound –directed transgastric ERCP for management of severe hepatic steatosis and fibrosis in a liver transplantation patient with bypass anatomy
A 54-year-old woman who had previously undergone Roux-en-Y gastric bypass presented with cholangitis 1 month after liver transplantation for alcohol-associated cirrhosis. Percutaneous transhepatic biliary drainage (PTBD) was performed for management of a suspected anastomotic stricture. Subsequent capping trials were unsuccessful, and the patient underwent placement of an internal-external percutaneous drain for several months, averaging 600 mL of bile drainage daily. A strategy combining endoscopic hand-suturing with clips for closure of rectal defects after endoscopic submucosal dissection with or without myectomy (with video)
Endoscopic hand-suturing (EHS) has been preliminarily demonstrated to be effective in closing defects after endoscopic submucosal dissection (ESD), but it is not easily performed. We proposed a strategy combining EHS with clips (EHS-Clips) and explored its effectiveness in closing rectal defects after ESD or ESD with myectomy (ESD-ME). (Source: Gastrointestinal Endoscopy)

Comparison of adenoma miss rate and adenoma detection rate between conventional colonoscopy and colonoscopy with second generation distal attachment cuff: a multicenter, randomized, back-to-back trial
Endocuff VisionTM has been designed to enhance mucosal visualization thereby improving detection of (pre-)malignant colorectal lesions. This multicenter, international, back-to-back, randomized colonoscopy trial compared adenoma detection rate (ADR) and adenoma miss rate (AMR) between Endocuff Vision-assisted colonoscopy (EVC) and conventional colonoscopy (CC). Validation of a new optical diagnosis training module to improve dysplasia characterization in inflammatory bowel disease: a multicenter international study
Inflammatory bowel disease (IBD) increases risk of dysplasia and colorectal cancer. Advanced endoscopic techniques allow for the detection and characterization of IBD dysplastic lesions, but specialized training is not widely available. We aim to develop and validate an online training platform to improve the detection and characterization of colonic lesions in IBD: OPTIC-IBD. (Source: Gastrointestinal Endoscopy)
