Transient Elastography and Serum-based Tests for Diagnosis of Fatty Liver and Advanced Fibrosis in a Community Cohort- a Cross Sectional Analysis
Background: Non-invasive tests (NITs) are necessary for knowing the true prevalence of fatty liver (FL) and advanced fibrosis (AF). Noninvasive tests (NITs) for diagnosis of FL and fibrosis were compared. Methods: Data were obtained from the National Health and Examination Survey (NHANES; 2017-2018). Participants were excluded with other liver diseases, missing data for NIT calculation and/or excessive alcohol use. Area under the receiver operating characteristic (AUROC) compared the accuracy of 4 FL NITs (CAP, HIS, FLI, USFLI) among themselves and to CAP value of 285 dB/m and 5 fibrosis NITs (transient elastography, APRI...

Molecular determinants of peritoneal dissemination in gastric adenocarcinoma
Peritoneal dissemination represents a poor prognostic indicator in gastric cancer. Despite a comprehensive molecular characterization of this disease, no peritoneal dissemination-specific signature has been identified, limiting the tailoring of the surgical and oncological treatments. In this review, we outline the available literature focusing on the role of the different molecular pathways involved in the acquisition of peritoneal metastatic dissemination. According to our results, several molecular determinants are associated with peritoneal carcinomatosis and are involved in several cellular and molecular carcinogeneti...

Validation and comparison of the Japanese score and other scores in patients with non-variceal upper gastrointestinal bleeding: a retrospective study
Conclusion: The Japanese score did not prove useful in Koreans with upper gastrointestinal bleeding. Additional research is needed due to the limitations of a retrospective study conducted in a single research institute. (Source: Digestive Diseases)

Histopathological Landscape of Precursor Lesions of Gastro-Entero-Pancreatic Neuroendocrine Neoplasms
Background: Despite the important advances in research on neuroendocrine neoplasms of the gastro-entero-pancreatic tract, their precursor lesions are much less well known.Summary: This review analyzes the preneoplastic neuroendocrine lesions of the gastro-entero-pancreatic tract, by adopting a coherent anatomical benchmark. In particular, the settings in which neuroendocrine precursor lesions represent well-recognized pathophysiological and morphological entities (with eventual molecular correlates) have been distinguished from the ones in which the nature of preneoplastic changes is still obscure.Key Messages: The aim of ...

The Histopathological Landscape of Precursor Lesions of Gastro-entero-pancreatic Neuroendocrine Neoplasms
Background. Despite the important advances in research on neuroendocrine neoplasms of the gastro-entero-pancreatic tract (GEP-NENs), their precursor lesions are much less well known. Summary. This review analyzes the pre-neoplastic neuroendocrine lesions of the gastroentro-pancreatic tract, by adopting a coherent anatomical benchmark. In particular, the settings in which neuroendocrine precursor lesions represent well recognized pathophysiological and morphological entities (with eventual molecular correlates) have been distinguished from the ones in which the nature of preneoplastic changes is still obscure. Glomus tumor of the stomach: a systematic review and illustrative case report
Discussion Epigast ric pain and bleeding were the most common symptoms at presentation in patients with diagnosis of glomus tumor. EUS-guided fine needle aspiration can be useful for preoperative diagnosis. Endoscopic elastosonography is a promising tool for the differential diagnosis of gastric submucosal lesions, in cluding glomus tumors. Treatment of choice is wedge resection with adequate free margins. Laparoscopic ap-proach is warranted when technically feasible. Since malignant gastric glomus tumors have been de-scribed, long term follow-up is suggested after surgical excision. (Source: Digestive Diseases)

Small-for-size syndrome and graft inflow modulation techniques in liver transplantation
Background: Partial liver transplantation (PLT) has recently been proposed to alleviate organ shortages. However, transplantation of a small-for-size graft (SFSG) is associated with an increased risk of post-transplant hepatic dysfunction, commonly referred to as small-for-size syndrome (SFSS). This review describes the etiology, pathological features, clinical manifestations, and diagnostic criteria of SFSS. Moreover, we summarize strategies to improve graft function, focusing on graft inflow modulation (GIM) techniques. Finally, unmet needs and future perspectives are discussed. Summary: In fact, post-transplant SFSS ca...

Simulation of aerosol and droplet spread during upper airway and gastrointestinal endoscopy
Conclusion: A simple and cheap drape has the potential to reduce aerosol and droplet spread during endoscopy. In terms of health care worker protection, this may be important particularly in low- or moderate-income countries. (Source: Digestive Diseases)

Referral for Dietary Intervention in Celiac Disease is Low Among Gastroenterologists and Primary Care Providers
Background: Strict adherence to a gluten free diet is the only known effective treatment for celiac disease currently. Multiple organizations recommend follow-up with a dietitian and guideline directed management after diagnosis. Few studies have evaluated follow-up post diagnosis. However, these do not include a systematic process for monitoring dietary referral among celiac disease patients. We sought to evaluate and compare the frequency of early dietary referral and guideline-directed preventive care and management for celiac disease patients managed by gastroenterologists and primary care providers. Methods: A re...

Ampullary Neuroendocrine Neoplasm: Clinicopathological Characteristics and Novel Endoscopic Entity
This study aimed to elucidate the clinical characteristics of ampullary NEN. Methods: We enrolled 162 patients with PB-NEN diagnosed at Kyushu University Hospital between 2011 and 2020. Clinical features, pathological diagnoses, treatments, and prognoses were retrospectively analyzed. We also compared ampullary NEN with pancreatic NEN (PanNEN). Results: We analyzed 10 ampullary NEN cases and 149 PanNEN cases. The ampullary NEN cases consisted of four cases of NET G1 (neuroendocrine tumor Grade 1), one NET G2 (Grade 2), and five NECs (neuroendocrine carcinoma). The incidences of NEC and cholangitis were significantly high...
