CONCLUSION: Unexplored conceptual, empirical and ethical issues surrounding open door policies and treatment pressures should be clarified by means of psychiatric and ethical research. [Impact of the Zeitgeist and human rights on psychiatric actions].
DISCUSSION: Human rights are a regulative idea that provide a framework and direction for psychiatric actions; however, as an idea they compete with other ideas that try to control the trends of the day (the Zeitgeist) that determine daily practice. Not only the suppression of human rights from public consciousness, e. g. with the eugenics in the first third of the previous century but also their absolutization and poor implementation, as in the Italian psychiatry reform of 1978, damage psychiatric patients. Therefore, it appears necessary to sensitize psychiatrists to reflect on the Zeitgeist and its influence...

[Autonomy focusing as guiding idea of minimally restrictive psychiatry].
r T Abstract Coercion and violence in psychiatric inpatient settings represent a serious challenge. This challenge cannot be successfully met just by denying any legitimation of coercive measures or by assuming that violence from the patients' side is a fateful complication. Coercive measures are essentially approximative solutions of interpersonal and intrapersonal conflicts of autonomy, if these cannot be resolved without violence. Therefore, prevention of coercive measures can only succeed when the focus is on the needs and rights for autonomy of all subjects involved, including patients, relatives an...

[Open doors, open mind : Plea for an open treatment psychiatry].
Abstract Patients who are committed against their natural will are regularly accommodated in closed wards in Germany. Only a few clinics treat patients who have been involuntarily committed on open wards. On the basis of own clinical experience and relevant study results, closed and open wards are compared with respect to the extent of aggression and coercive measures, the risks of absconding and suicide. Compared to open wards, closed wards are unlikely to reduce the risk of absconding, do not result in a decreased suicide rate and do not result in a reduction of aggressive behavior. In contrast, c...

[Pituitary adenomas].
Abstract Despite characteristic symptoms the diagnosis of clinically relevant pituitary adenomas is often delayed until an advanced stage due to the rarity of the disease. The typical clinical manifestations are presented in this review article. The recent discovery of the USP8 mutation in Cushing's disease and of X‑linked acrogigantism (X-LAG) syndrome in early onset gigantism were milestones in the search for the molecular etiology of pituitary adenomas. The triad of endocrinological, radiological and ophthalmological diagnostics are the main pillars for the diagnostic work-up of pituitary adenomas. The standa...

[The CIMH track concept in the treatment of psychotic disorders].
g A Abstract The treatment of psychotic disorders and illnesses is a challenge for therapists and institutions due to the heterogeneity of the cause and course, refractory symptoms, lack of therapy adherence and high rates of relapse. These circumstances can be effectively counteracted by the flexibility of therapeutic approaches and settings. A useful but rarely used concept is the treatment of psychoses within the so-called track unit. A track unit is defined as a syndrome-oriented, decentralized, modular unit, adjusted to the patient's individual stage-specific needs across both inpatient an...

[Autoantibody-associated schizophreniform psychoses: clinical symptomatology].

In contrast, secondary psychotic disorders are the result of likely identifiable organic factors either in terms of a first causation (etiology) or a secondary cause (pathogenesis). In this context, autoantibody(ab)-associated autoimmune encephalitis (AE) plays an increasingly important role. Within the group of ab-associated AE with neuropsychiatric symptoms, anti-N-methyl-D-...

[Track treatment in psychiatry: the CIMH track model to overcome sector boundaries].
Abstract The overall conditions for the treatment of patients with psychiatric disorders have changed and place new demands on therapy concepts and procedures. This concerns both the legal conditions of the treatment as well as the content and economic conditions for a patient-oriented diagnostic work-up and therapy. The Central Institute for Mental Health (CIMH) in Mannheim is currently implementing a track concept that takes these changes into account and aims to overcome sectoral boundaries in favor of treatment continuity. Abstract Acute management as well as establishing the etiology of an intracerebral hemorrhage is still a challenge for clinicians. Besides discussing the classification of intracerebral hemorrhages, this article focuses on a diagnostic algorithm as well as the presentation of potential (rarer) causes of non-traumatic intracerebral hemorrhages.
