Management of treatment resistance-Treatment-resistant schizophrenia
Nervenarzt. 2024 Feb 6. doi: 10.1007/s00115-024-01608-6. Online ahead of print.ABSTRACTDespite a very high prevalence and substantial impairments among affected individuals, treatment-resistant schizophrenia (TRS) has not been sufficiently researched in clinical research in the field of psychiatric disorders and the pathophysiology is still poorly understood. A better clinical and pathophysiological understanding of this heterogeneous and severely affected population of people with persistent symptoms in different domains is necessary in order not only to be able to intervene early but also to develop novel therapeutic str...

Transdisciplinary treatment of functional movement disorders: integration instead of dissociation
Nervenarzt. 2024 Feb 5. doi: 10.1007/s00115-023-01596-z. Online ahead of print.ABSTRACTSuccessful treatment of patients with functional motor disorders is integrative in several ways: the primary treatment goal is the (re)integration of sensorimotor, cognitive and social functioning. The prerequisites for this are an integrated biopsychosocial model of everyone involved as well as close transdisciplinary cooperation. Instead of a simple addition of treatment components, all care providers and patients act in concert.PMID:38315181 | DOI:10.1007/s00115-023-01596-z (Source: Der Nervenarzt)

Migraine and mitochondrial diseases : Energy deficit in the brain
Nervenarzt. 2024 Feb;95(2):169-178. doi: 10.1007/s00115-024-01622-8.ABSTRACTMitochondrial diseases are complex metabolic disorders caused by genetic mutations and lead to impaired energy production in the mitochondria of cells. The clinical spectrum ranges from severe multiorgan involvement in early childhood to mild monosymptomatic courses in adulthood. The brain, heart, and skeletal muscles are particularly affected due to their high energy demands. Headaches in general and migraine in particular, occur disproportionately more frequently in patients with mitochondrial diseases. In recent years similarities in the pathome...

Virtual reality in teaching using mentally ill patient avatars
CONCLUSION: The AVEX project already shows promising possibilities for supplementing the teaching of medical students, even if the fit of questions and replies in dialogue with the virtual avatars still needs to be improved. As advances in the linguistic communication of emotions and the visual effects of the avatar representation can be predicted, the significance of this technology will continue to increase.PMID:38277046 | DOI:10.1007/s00115-024-01610-y (Source: Der Nervenarzt)

Curriculum "Tracheostomy management in dysphagia therapy"
Nervenarzt. 2024 Jan 26. doi: 10.1007/s00115-023-01598-x. Online ahead of print.ABSTRACTThe number of tracheotomized patients with dysphagia and their need for treatment are continuously increasing in clinical and community settings. The revised version of the directive on home care and community-based intensive care of the Federal Joint Committee (G-BA) requires that tracheotomized patients are regularly evaluated with the aim of identifying and promoting the therapeutic potential after hospital discharge. Dysphagia treatment plays a crucial role as without improvement of severe dysphagia there is practically no possibili...

Efficacy of internet-based interventions for depression available in Germany-A  systematic review and meta-analysis
CONCLUSION: The IBIs for depressive disorders available in Germany are effective and can therefore be used in the treatment of people with a depressive disorder; however, it is possible that not all interventions are equally effective.PMID:38260995 | DOI:10.1007/s00115-023-01587-0 (Source: Der Nervenarzt)

Incidence of sudden death in epilepsy (SUDEP): update and limitations
CONCLUSION: Considering all age groups and types of epilepsy it is estimated that about 1 in 1000 individuals with epilepsy dies of SUDEP annually. With an assumed epilepsy prevalence of 0.6% in Germany, this could lead to more than one SUDEP case daily. Standardization of research methods is essential to gain more profound insights.PMID:38252160 | DOI:10.1007/s00115-023-01595-0 (Source: Der Nervenarzt)

Telemedicine and international projects: from Asia to Africa: chances for the future?
Nervenarzt. 2024 Jan 19. doi: 10.1007/s00115-023-01605-1. Online ahead of print.ABSTRACTAlthough the morbidity and mortality of neurological diseases in many Asian and African countries is high and are predicted to increase even further in the coming decades, in many areas there is a shortage of medical personnel and high-quality treatment options. This shortage, together with a frequently insufficient healthcare infrastructure, limits the access of many patients to medical treatment. The possibilities of telemedicine are multifarious. It provides new, so far unused possibilities in the diagnostics and treatment of neurolo...
