Use of allergy tests to identify dietary and environmental triggers of eosinophilic esophagitis: protocol for a  systematic review
SummaryPurposeEosinophilic esophagitis (EoE) is a  chronic immune-mediated esophageal disease. Most EoE patients have atopic comorbidities and they test positive when allergy tests are performed. Our aim is to critically assess the relation of allergens detected by allergy tests with clinically confirmed triggers of EoE.MethodsA  systemic review and meta-analysis will be conducted, which will involve searching electronic databases using predetermined search terms, in order to identify relevant studies. Studies will be independently screened against predefined eligibility criteria and subgroup analysis will be used for th...

Incidence rate of severe adverse drug reactions to nonionic contrast media at the National Hospital Organization Osaka National Hospital
DiscussionAlthough the rate of severe adverse drug reactions in this study appears to be higher than expected, the sample size of 42,501 patients is insufficient to conclusively determine the incidence and mortality rates of adverse drug reactions, as these rates are extremely low. Further studies are required to estimate the countrywide prevalence of adverse drug reactions in response to contrast media. (Source: Allergo Journal International)

Is the concept of “peanut-free schools” useful in the routine management of peanut-allergic children at risk of anaphylaxis?
ConclusionIn order to address the fears and concerns of patients, parents, and school staff, it is mandatory to develop various coping strategies. These should enable and ensure the safety and participation of food-allergic pupils in classes and other school activities. Therefore, it is important to implement adequate measures for allergen avoidance and emergency treatment for students with confirmed food allergies. (Source: Allergo Journal International)

Suspected penicillin allergy: risk assessment using an algorithm as an antibiotic stewardship project
ConclusionTriage of patients with suspected BLA is urgently required. While allergy testing, including provocation testing, represents the most reliable solution, this is not feasible in all patients due to the high prevalence of BLA allergies. The risk stratification algorithm developed for the authors ’ hospital represents a tool suitable to making a contribution to rational antibiotic therapy. (Source: Allergo Journal International)

Therapy of allergic rhinitis in routine care: evidence-based benefit assessment of freely combined use of various active ingredients
ConclusionThe free combination therapies of mast cell stabilizers, decongestants, AHs and nasal GCs, frequently used in the routine care of patients with AR, cannot be recommended because they are not evidence-based. Due to the fact that over-the-counter antiallergic drugs are not reimbursable in Germany, there is no medical supervision of the therapy. In addition, there are doubts about appropriate treatment, especially of patients with persistent rhinitis with severe symptoms, as these patients often use several preparations at the same time to alleviate their symptoms. Identification of a  53 kDa protein, as a new high molecular weight allergen from Fraxinus excelsior (Ash) pollen
In this study, a high molecular weight allergen from ash pollen was identified.MethodsIn all, 20 individuals allergic to ash participated in the study. Characterization and immunoreactivity of ash pollen proteins was performed using sodium dodecyl sulfate polyacrylamide electrophoresis (SDS-PAGE), two-dimensional (2D) gel electrophoresis and immunoblotting.ResultsImmunoglobulin E (IgE)-binding proteins with apparent molecular mass ranging from 9  to 110 kDa were detected in ash pollen extract. Serum IgE of 7 (35%) patients reacted with a 53-kDa protein band. Analysis of 2D immunoblots showed several IgE-binding prote...

Hypersensitivity reactions to biologics (part  II): classifications and current diagnostic and treatment approaches
ConclusionHSRs to BS are gradually increasing with the widening of their clinical use and indications. It is very important to prevent HSRs and to know the degree of severity as well as the emergency treatment algorithm. This review summarizes the diagnostic tests that should be applied: (a)  immediately during/after a reaction, and (b) subsequently, and in the case that a switch of BS is not possible, desensitization is an option. Omalizumab in three children with severe vernal keratoconjunctivitis
ConclusionAlthough the clinical benefit of omalizumab in asthma and chronic spontaneous urticaria (CSU) has been established in several clinical trials, there are very little data about its effect on severe VKC. In addition to few previously reported cases we can report the rapid effectiveness of omalizumab in VKC clinically and in terms of quality of life. Randomized trials are needed to include omalizumab in third-line treatment of VKC for prevention of visual impairment and further sequelae such as corneal damage.
