Case Report: Delayed Diffuse Alveolar Hemorrhage in Echis sochureki Envenoming, Jodhpur, India
Am J Trop Med Hyg. 2022 Jan 31:tpmd211187. doi: 10.4269/ajtmh.21-1187. Online ahead of print.ABSTRACTSnakebite envenoming is a common occupational hazard in the tropics. Venom-induced consumption coagulopathy and acute kidney injury are the most frequently encountered complications of viper bites. Diffuse alveolar hemorrhage (DAH) is an unusual presentation reported rarely in the literature. Our case report highlights the uncommon presentation of delayed pulmonary hemorrhage after snakebite envenoming. A 40-year-old healthy man presented to our emergency department after 6 hours of Echis sochureki (a saw-scaled viper subsp...

Mutations Accounting for Congenital Fibrinogen Disorders: An Update
Semin Thromb Hemost DOI: 10.1055/s-0041-1742170Fibrinogen is a complex protein that plays a key role in the blood clotting process. It is a hexamer composed of two copies of three distinct chains: Aα, Bβ, and γ encoded by three genes, FGA, FGB, and FGG, clustered on the long arm of chromosome 4. Congenital fibrinogen disorders (CFDs) are divided into qualitative deficiencies (dysfibrinogenemia, hypodysfibrinogenemia) in which the mutant fibrinogen molecule is present in the circulation and quantitative deficiencies (afibrinogenemia, hypofibrinogenemia) with no mutant molecule present in the bloodstream. Phenotypic manif...

Impact of fibrinogen infusion on thrombin generation and fibrin clot structure in patients with inherited afibrinogenemia
CONCLUSIONS: In summary, fibrinogen infusion with a standard dose of FC increased but did not correct TG and led to formation of fibrin clots similar to those of patients with hypofibrinogenemia. All in all, our results do not support to biological evidence of hypercoagulability induced by FC in patients with afibrinogenemia.PMID:35045578 | DOI:10.1055/a-1745-0420 (Source: Thrombosis and Haemostasis)

Management of cytokine release syndrome and macrophage activation syndrome following CAR-T cell therapy: Guidelines from the Francophone Society of Bone Marrow Transplantation and Cellular Therapy (SFGM-TC)
Bull Cancer. 2021 Dec 8:S0007-4551(21)00447-1. doi: 10.1016/j.bulcan.2021.11.002. Online ahead of print.ABSTRACTThe use of chimeric antigen receptor T cells (CAR-T) has increased since their approval in the treatment of several relapsed/refractory B cell malignancies. The management of their specific toxicities, such as cytokine release syndrome (CRS), tends to be better understood and well-defined. During the twelfth edition of practice harmonization workshops of the Francophone Society of Bone Marrow Transplantation and Cellular Therapy (SFGM-TC), a working group focused its work on the management of patients developing ...

How to recognize and manage COVID-19-associated coagulopathy
Hematology Am Soc Hematol Educ Program. 2021 Dec 10;2021(1):614-620. doi: 10.1182/hematology.2021000297.ABSTRACTCOVID-19 is frequently associated with abnormalities on coagulation testing and a coagulopathy driven by inflammation, intravascular coagulation activation, and microvascular thrombosis. Elevated D-dimer is the most common finding and is a predictor of adverse outcomes including thrombosis, critical illness, and death. Although COVID-19-associated coagulopathy has some similarities to disseminated intravascular coagulation, the platelet count is usually preserved, coagulation times are usually normal or minimally...
