Comprehensive analysis of cytokine adsorption properties of polymethyl methacrylate (PMMA) membrane material
In this study, we comprehensively analyzed the adsorption of 48 different cytokines in human plasma to PMMA and polysulfone (PS) membranes. Seventy-nine percent (38/48) of the cytokines were adsorbed more efficiently to the PMMA membrane than the PS membrane, which indicates that the PMMA membrane has high cytokine adsorption ability. The adsorption rate tended to be higher for the cytokines with lower molecular weight, and there was a significant correlation between the molecular weight of the cytokines and the adsorption rates. Electron microscopy showed that the PMMA hollow fiber membrane had a uniform internal structur...

Advanced thallium toxicity
We report a 41-year-old male landscaper admitted for acute polyneuropathy and abdominal pain. He was treated for suspected Guillain-Barré syndrome and later autoimmune encephalopathy. However, over the next 42 days, he developed worsening muscle weakness, delirium and alopecia, and was diagnosed with thallium toxicity. After combining Prussian blue, activated charcoal and continuous venovenous haemofiltration, he improved though with neuropsychiatric and neuromuscular sequelae. We highlight the need to manage information disclosure properly and to preserve evidence, when the source of a toxin is unclear.PMID:36424143 | DO...

Strategies to mitigate inflammation in management of complex congenital heart disease complicated by & #8220;multisystem inflammatory syndrome in children & #8221;
Anil Kumar, Reena K Joshi, Neeraj Aggarwal, Mily Ray, Raja JoshiAnnals of Pediatric Cardiology 2022 15(3):276-279 A 6-month-old boy, a case of Shone's complex, presented in decompensated state was found to have severe mitral stenosis along with multisystem inflammatory syndrome in children (MISC) warranting urgent surgical intervention. Various modalities including cytokine-adsorbing hemofilter were used to target inflammation. Postoperatively, the child recovered from low cardiac output accompanied by decrease in the levels of inflammatory markers, inopressors, and ventilatory requirements. Open heart surgery ...

Early and long-term outcomes of conventional and valve-sparing aortic root replacement
Conclusions ARR and VSRR can be performed with low mortality and morbidity as well as a low rate of reintervention during the period of long-term follow-up, if performed by an experienced team with a consistent perioperative approach. This series provides contemporary evidence to balance the risks of aortic aneurysms and their rupture at diameters of <5.5 cm against the risks and benefits of surgery. (Source: Heart)

Negative calcium balance despite normal plasma ionized calcium concentrations during citrate anticoagulated continuous venovenous hemofiltration (CVVH) in ICU patients
CONCLUSION: During citrate CVVH, calcium balance was negative in most patients, despite supplementation of calcium based on plasma ionized calcium levels. This may contribute to demineralization of the skeleton. We propose that calcium supplementation should be based on both plasma ionized calcium and a simple calculation of calcium excretion by CVVH.PMID:36342643 | DOI:10.1007/s40620-022-01482-y (Source: Journal of Nephrology)

Delayed olanzapine induced myocarditis and rhabdomyolysis
A 65-year-old woman with a background of paranoid schizophrenia presented to hospital with generalised muscle aches and weakness. She was diagnosed with schizophrenia 2 years prior following an acute psychotic episode and was initiated on olanzapine 10 mg (once nightly). She was fully compliant with treatment and not prescribed any other medications. On admission, she was afebrile with a normal blood pressure and oxygen saturations on room air. Initial assessment demonstrated reduced power (4/5) and muscle tenderness bilaterally in her deltoids and quadriceps, and she was passing minimal amounts of dark urine. Blood tests ...
