A Randomized Controlled Trial of the Implementation of BREASTChoice, a Multilevel Breast Reconstruction Decision Support Tool With Personalized Risk Predictiona
CONCLUSIONS: BREASTChoice is a novel decision tool incorporating risk prediction, patient education, and clinician engagement. Patients using BREASTChoice had higher knowledge; older adults and those from racially minoritized backgrounds especially benefitted. There was no impact on other decision outcomes. Future studies should overcome implementation barriers and specifically examine decision outcomes among high-risk patients.PMID:38801247 | DOI:10.1097/SLA.0000000000006354 (Source: Cancer Control)

Use of CYP2D6 Inhibitors with CYP2D6 Opioids: Association with Emergency Department Visits for Pain
Clin Pharmacol Ther. 2024 May 26. doi: 10.1002/cpt.3314. Online ahead of print.ABSTRACTHydrocodone, tramadol, codeine, and oxycodone are commonly prescribed opioids that rely on activation by cytochrome P450 2D6 (CYP2D6). CYP2D6 inhibitors can significantly decrease CYP2D6 activity, leading to reduced generation of active metabolites, and impairing pain control. To understand this impact, we assessed emergency department (ED) visits in patients initiating these CYP2D6-dependent opioids while on CYP2D6-inhibitor antidepressants vs. antidepressants that do not inhibit CYP2D6. This retrospective cohort study included adult pa...

Risk factors associated with escalation of care in a quaternary academic hospital at home program
AbstractBackgroundHospital-at-home has become a more recognized way to care for patients requiring inpatient hospitalization. At times, these patients may require escalation of care (transfer from home back to the brick-and mortar (BAM) hospital for ongoing hospitalization care needs), a process that has not been extensively studied.ObjectiveTo evaluate what patient factors contribute to escalations of care in the hospital-at-home delivery model.Designs, Settings, and ParticipantsWe conducted a retrospective review of all patients admitted to Mayo Clinic's Advanced Care at Home (ACH) program from January 1, 2022 to Decembe...

Prognostic clinical decision support for pneumonia in the emergency department: A randomized trial
ConclusionsEHR-based prognostic CDS did not improve ED disposition decisions for children with pneumonia. Although the intervention's content was favorably received, low subject accrual and workflow integration problems likely limited effectiveness.  Clinical Trials Registration: NCT06033079. African American patients have a higher probability of cognitive impairment after incident stroke: An analysis of national electronic health record data
Cognitive impairment (CI) and stroke are diseases with significant disparities in race and geography. Post stroke cognitive impairment (PSCI) can be as high as 15-70% but few studies have utilized large administrative or electronic health records (EHR) to evaluate trends in PSCI. We utilized an EHR database to evaluate for disparities in PSCI in a large sample of patients after first recorded stroke to evaluate for disparities in race. Comprehensive ABC (HbA < sub > 1c < /sub > , blood pressure, LDL-C) control and cardiovascular disease risk in patients with type 2 diabetes mellitus and major depressive disorder in a South African managed healthcare organisation
CONCLUSIONS: Despite a managed-care environment, the comprehensive ABC control among patients with T2DM was suboptimal, particularly in those with MDD, placing them at greater risk for CVD events.PMID:38787623 | DOI:10.5830/CVJA-2024-003 (Source: Cardiovascular Journal of Africa)

Care Quality and Equity in Health Centers During and After the COVID-19 Pandemic
CONCLUSIONS: While preventive screening and chronic disease management in FQHCs have largely rebounded to pre-pandemic levels following an initial decline, persistent disparities highlight the need for targeted interventions to support FQHCs in addressing healthcare inequities.PMID:38788862 | DOI:10.1016/j.amepre.2024.05.011 (Source: Cancer Control)

Development and Implementation of an Integrated Standard e-Prescription Model in Alignment with Iranian National EHR
Stud Health Technol Inform. 2024 May 23;314:139-143. doi: 10.3233/SHTI240077.ABSTRACTThe implementation of an Electronic Prescribing (EP) system offers numerous advantages in enhancing the efficiency of prescribing practices. To ensure successful implementation, a comprehensive understanding of the workflow in paper-based prescribing is crucial. In Iran, the Ministry of Health, and Medical Education (MOHME) has been actively involved in developing an EP system since 2011. The pilot results within MOHME have garnered significant support from all basic insurance organizations, primarily due to the importance of addressing fi...

Exploring Negated Entites for Named Entity Recognition in Italian Lung Cancer Clinical Reports
Stud Health Technol Inform. 2024 May 23;314:98-102. doi: 10.3233/SHTI240066.ABSTRACTThis paper explores the potential of leveraging electronic health records (EHRs) for personalized health research through the application of artificial intelligence (AI) techniques, specifically Named Entity Recognition (NER). By extracting crucial patient information from clinical texts, including diagnoses, medications, symptoms, and lab tests, AI facilitates the rapid identification of relevant data, paving the way for future care paradigms. The study focuses on Non-small cell lung cancer (NSCLC) in Italian clinical notes, introducing a ...
