Pregnancy outcomes from controlled ovarian hyperstimulation in overweight and obese patients without pcos treated with letrozole versus clomiphene citrate
Anovulatory cycles due to gonadotropin dysfunction contribute to infertility in women with polycystic ovary syndrome (PCOS). Women with PCOS have significantly higher luteinizing hormone (LH) pulse amplitude and frequency compared to women without PCOS(1). However, body mass index (BMI) was significantly negatively correlated with LH pulse amplitude in PCOS and non-PCOS women(1). Even in obese ovulatory women, LH secretion was reduced due to impaired LH pulse amplitude (2). Clomiphene citrate throughout the duration of ovarian stimulation: an approach to decrease costs, reduce injection burden, and prevent premature ovulation in patients with diminished ovarian reserve
Clomiphene citrate (CC) has been widely utilized as an adjunct in ovarian stimulation protocols for in-vitro fertilization (IVF) to capitalize on pituitary follicle-stimulating hormone (FSH) release in patients with diminished ovarian reserve (DOR). In these protocols, CC is typically administered with gonadotropins for the first five days followed by GnRH antagonist to prevent premature ovulation. However, due to its antiestrogenic mechanism of action at the pituitary, CC may also effectively prevent the LH surge if continued until day of trigger. (Source: Fertility and Sterility)

Oral sildenafil citrate: a potential approach for improvement of endometrial thickness and treatment of unexplained infertility in women
