27 yr old female comes to the clinic as she was unable get pregnant from 1 year despite regular sexual encounter . She complains of irregular periods varying from 35- 42 days lasting for 8 days. O/e pelvis and adnexa normal , HSG reveled patent Fallopian tubes . Her husband had 2 children from the previous marriage and post coital semen analysis in normal now . Unorthodox Treatments
I graduated less that a year ago, and getting used to practicing on my own. I still ask a lot of advice from other providers at the clinic, as well as look up a ton of stuff on up-to-date. I've seen some interesting treatments from the other providers that up-to-date has nothing on. For example, using Clomid for low testosterone or using nitro or isosorbide for pvd. REI: clomid/IUI - trigger or no trigger that is the question
I was reading about Clomid/IUI on UpToDate about this for a friend and came across the following info that basically states natural LH monitoring + IUI is more effective than hCG trigger + IUI in Clomid cycles. However it seems to me that most centers -- including my med school and residency -- do use an hCG trigger! Why is this evidence being ignored? What does your center do? I'd like to get some feedback from anyone that has gone through IUI with femara or injectables and had success. My husband and I have been struggling with infertility for over 3 years now. Not only is my CF an issue, but he also has mild male factor. We did 2 IUIs in 2013 that were unsuccessful and sought a 2nd opinion and at that time was told that IVF w/ICSI was the way to go. Well, turns out, unless I can find an IVF clinic that will do the retrieval in an OR, IVF will not be an option for us.
