Chronic testicular pain

Intralipid Availability
I just graduated from fellowship this last June and have started at a private practice gig where most of the procedures short of implants are done in office. I am anesthesia trained and it has been emphasized to me since residency to have Intralipid available and know where it is at any location I will be performing blocks. Academics vs PP...how to choose?
If I am unsure of whether I would enjoy academics or private practice more, which should I start out in? Is it easier to move from PP to academics or vice versa? I imagine I would stick with a situation for at least 2-3 years before trying something new. Can anyone speak to their experiences and satisfaction regarding either setting, especially those who have done both? I am interested in hearing from OR anesthesiologists and chronic pain doctors. Why the hell do PCPs refer podiatrists patients with Chronic gout? It’s called medical management Dr. PCP MD. What am I going to offer the patient? Do you guys think there is enough volume to support an outpatient interventional cancer pain & cancer rehab practice? I will be starting my PM&R residency training at one of the top programs in the country after this academic year (am an prelim medicine intern right now). I’ve been seeing a lot of patient on Belbuca for chronic pain. 1. do you guys think belbuca is more efficacious or lower risk than just using conventional opioids for chronic pain. 2. I’ve seen some people prescribing big doses of Belbuca, and concurrently prescribing immediate release oral opioids for breakthrough pain.
