TAP block consent
I've had it come up a few times where I didn't consent pre-op for TAP block but surgery takes an unexpected course and now they have an unanticipated abdominal incision and the patient really could benefit from a TAP block prior to emergence at this point. Is it reasonable to perform the block? They are so safe, I do them under GA anyway, and the benefits if the person has significant chronic pain issues would be potentially great. Chronic pain
90 year old patient with chronic low back (prior lami/fusion) and continued knee pain after TKA. I’m the 3rd PM doc she’s seen. She failed to follow up with the other PM bc she says nothing was helping. per chart review she’s been tried on multiple meds(meds all with side effects or no relief: gaba, Lyrica, cymbalta, savella, Tylenol #3, Vicodin, oxycodone) I’m hesitant to try TCA at her age and she has afibb. Buprenorphine for pain
What is everyone's experience with buprenorphine for pain? I've only given butrans patches from 5 to 20 mcg and find that it isn't really strong enough for many patients with chronic pain. What about belbucca, and Subutex? Is the 2mg Subutex too much? (Source: Student Doctor Network)

Compression Fracture and Vertebral Augmentation
Usually I would only do Kyphos on patients with an acute/subacute MRI with edema on the fracture. I recently saw a patient with a CHRONIC compression fracture. But it is very tender.
