Pediatric Pain Medicine Fellowships
Anyone have any insight into this field, it seems relatively new with only afew institutions offering the fellowship. Do you receive a general certificate as a pain physician, or are you limited to only pediatric pain management? What is the caseload? Terminal pediatric patients? Procedural heavy? I love working with kids but I'm wondering if there is a similar lifestyle to adult chronic pain practice? - important to consider every aspect of the field before committing to it. Interesting Case
Had a patient today in clinic, elderly female, history of lumbar lami L3-S1 for leg pain 6 months ago. She came to me today because of continued post op leg pain (RLE > LLE), unable to walk now after surgery. Surgeons wont touch her. Neurologists don't know. EMG shows chronic L5 radic, but otherwise normal. MRI with residual L5 NF stenosis. MAC Anesthesia with chronic pain pts
I was wondering since my interest is in chronic pain. Does the presence of chronic pain change the way an anesthesiologist treats the pt? Especially with MAC anesthesia when the pt is awake and sometimes the procedure would appear that the pt only needs local anesthetic, but because of comorbidities (ie, chronic pain) would make the procedure not tolerable? Exam Room 1: Consent for Elective Buprenorphine Rotation
78 year old female with advanced OA, spondylosis, multiple medical comorbidities and significant cognitive impairment residing in assisted living/memory care unit on FNT 50 mcg/hr TD patch and two percocet 5/325's QD. PCP no longer RX's opioids for chronic pain. Family, caregiver, and private duty RN want patient off "heroin pills" and fentanyl. Patient does not want to rotate. Significant family issues and litigated trust. Psychiatrist says patient is competent despite cognitive issues.... Pain and Rheum
Hey guys, PGY1 here, just finished a rheumatology rotation and noticed a lot of patient with chronic pain that would benefit from interventioinal pain or pharm treatment such as fibro, or inflammatory arthritis that could benefit from geniculat blocks, or chronic neck and back pain. I am interested in pain, do not know all available interventions but noticed the patients were not well controlled at times. Chronic Foot Pain
This is a 45 year old patient with Charcot foot who has severe constant neuropathic pain. She uses high dose opioids to remain functional. I am tapering after inheriting this patient. Pain is flaring up. I am considering sympathetic nerve blocks. But would anyone do anything like phenol on the cutaneous nerves or RFA or something weird and experimental like that? Any other ideas?
