What percent of your patients are on chronic opioid therapy? Advice for new graduate?
Hello, I was hoping if someone could give some advice. I am a recent graduate that trained in a place where we were pretty much not prescribing any type of opioid medications. I would say only 5% of our patients were on chronic opioid medications. It seems like they screened a lot of patients and I was pretty shielded from certain patients. Chronic abdominal pain and nausea?
I'm taking care of an outpatient patient: 60y/o woman with chronic abdominal pain with prior history of gastric bypass, appendectomy, incisional hernia repair. Her pain had been previously controlled with oxycodone 10mg a couple times a day, but recently less effective due to emesis. GI is following now, has her on Zofran, Reglan, Promethazine but the nausea is still pretty severe. Patient is cachectic and having trouble with PO intake. Utox is appropriate. More buprenorphine stories
There seems to be a prevalence of people using or wanting to use buprenorphine more for chronic pain. Reasons PRO (some debatable): Less addictive Less prone to abuse Less euphoric Less tolerance Reverse tolerance Less respiratory depression (but same if on other respiratory depressants) Schedule 3 Doesn’t show up in popular music as far as I know.
