A1c and injections
what are your policies re this if they aren't going to get steroids? let's say they have A1c of 11+ would you do TPI? MBB? RFA? what if the story was complicated by chronic poorly healing wounds in the limbs? curious what people's thresholds are and what a1c you'd inject at. (Source: Student Doctor Network)

Why is there no pain in chronic pulpitis?
Hey everyone I am a dental student and this is a question that I am curious about. In acute pulpitis the patient feels severe pain so what is the reasoning that in chronic pulpitis the pain disappears even though the pulp is still inflamed? (Source: Student Doctor Network)

Do you need a fellowship?
Hi everyone! I have some questions that I hope generate a discussion. Hopefully, this helps current PM&R residents and medical students considering PM&R. 1) For a PM&R resident, do you need a fellowship? 2) What can you do without a fellowship? 3) What must you do a fellowship in order to do? My personal feeling is that unless you want to be uber subspecialized in a particular patient population (ex. athletes, chronic pain, brain injury, SCI, kids), want to cover professional/Olympic teams... Chronic opioid management, clinic visit?
It could be a silly question but need some input. I took over the hospital-based pain clinic from an old pain doctor who was retiring. He saw chronic opioid patients every 3 months and he prescribed 30 days of medication every 28 days. He did a drug screen every 6 months but no pill counts. My understanding is that most private practice sees opioid patients every month and does pill counts. Do you recommend seeing opioid patients every month in a hospital pain practice setting as well... Read more (Source: Student Doctor Network)

Advice on mentioning misdiagnosis and alternative medicine on Personal Statement?
Hi, I’m currently in the process of writing my personal statement. A large part of my reason to go into medicine was that my sister had intense chronic pain that for a long period of time, she was diagnosed differently by multiple doctors and treatments were never effective. Our family physician didn’t give up on her case and eventually referred us to an alternative medicine practitioner (who practiced something similar to OMT) who was able to eliminate her pain. We never really understood... Read more (Source: Student Doctor Network)

Harm reduction and the chronic pain patient
I thought I understood the core tenets of harm reduction and strongly support them but I was “educated” on Twitter a bit ago about HR in setting of chronic pain. The CPPs and some HR providers feel it is inappropriate to have these patients to taper off/down on opiate doses or switch to MAT. HR principles support maintaining these patients on large doses of opiates because they are stable on them These are obviously the most vocal patients/advocates and they are the exception to the...
