Obligation to legacy patients
Have a doc in our practice that left for the VA. Was managing some chronic opiates. Not a lot of patients and most are on fairly reasonable amount of mme as he was working towards tapering. He was really the only one, for the most part, doing COT in our group. Because I'm newer to the group I was asked if I would take on management and because the other docs have zero interest. I reluctantly agreed but am not sure as to my obligation to their management. I'm in a state where marijuana is... Read more (Source: Student Doctor Network)

What would you do?
I have an 80 year old woman with osteoporosis and prior compression fractures. She developed significant acute axial low back pain February 18th without any inciting event. This was so severe that it resulted in a short hospitalization for pain control. Workup was negative except for chronic L3 and L4 compression fractures (I have included sagittal STIR and T2 lumbar spine MRI images below from 2/19/2023). Patient was referred to me in March. Pain was facetogenic on examination. I ended up... Read more (Source: Student Doctor Network)

Back pain management
I'm almost 2 years out of training, still have some doubts regarding managing nontraumatic lower back pain in the ER: Obviously in people with no red flags, treat symptomatically and discharge. The main problem are these patient who usually have some worsening of their chronic back pain, coming in with some mild neuro complaint like one leg is starting to feel numb or a little weak, or some numbness in the groin. Assuming infectious workup is negative and they get a MRI non-con, always... Read more (Source: Student Doctor Network)

Chronic Pain and the Differential Diagnosis of Mood Disorders
I need to get people's opinions on a differential diagnostic issue. Situation would be a patient with chronic pain who develops clinically significant depressive symptoms as a result (or, at least with the stress of the chronic pain contributing substantially to mood dysfunction). My contention would be that this would NOT be diagnosed as a 'Depressive Disorder Due to Another Medical Condition' but rather an Adjustment Disorder With Depressed Mood (or even MDD if full criteria for MDD are... New Study Shows Chronic Pain is Real After all...
Has Science Cracked the Code on Chronic Pain? In a first, researchers have identified chronic pain pathways in individuals directly from their neural activity, showing promising paths forward for treatment. www.ucsf.edu If I were Deyo, Chou, anyone who ever published an article in the Back Letter, or therapeutic nihilists/pain deniers, I'd be pissed... Average reimbursement for implants
Hi, Can anyone please let me know the average reimbursement you (physician, not hospital) get for doing an ITP or SCS implant at a hospital if you are working in PP? Do you generally negotiate your professional fee with the hospital, or is it normally a set price according to reimbursement? I want to do pumps for chronic malignant pain and stims, but if it is not economical then I will just make it a smaller part of my practice. How do you generally gauge what procedures are more "worth... Read more (Source: Student Doctor Network)

Prophylactic Antibiotics and Neuraxial
In fellowship, our institutional dogma was that if a patient was on antibiotics we would not perform any neuraxial intervention. Have patient on chronic antibiotics (6 months into 2.5 year course) for knee replacement hardware infection with a lumbar radicular symptom who could benefit from a LESI. No infectious symptoms right now. I spent some time reviewing the literature (google schoalr search) this morning and couldn't find anything that spoke to recommendations if a patient was on... TMS financial questions
I'm a psychiatry-matched MS4 currently on a pain clinic elective. I am giving a presentation about some RCTs of TMS for chronic pain conditions and would like to include some basics about reimbursements for TMS in general. I'm aware that it's not currently billable for pain (except migraines), but the goal is to provide some background about the reimbursement landscape for approved conditions. I've scoured some old threads and found the following info thus far: 2022 thread: ... Read more (Source: Student Doctor Network)
