benzos and amphetamines
As a pain specialist occasionally I see truly 'difficult' patients with some combination of depression, anxiety, maybe a PD, and, of course, chronic pain. Usually they are on ridiculous doses of opioids that barely "take the edge off", and among their meds you find both benzos and amphetamines. When I see this I have two reactions: 1) this is an incredibly manipulative patient, and 2) the psychiatrist is a weak fool, or just doesn't care. Ways to be 100% sure that a controlled substance prescription is legitimate?
I have 3 different scenarios: Patient A has been getting hydrocodone/apap 10-325mg #150, phentermine 37.5 mg #30, promethazine /codeine syrup 473 ml, and Soma 350mg #90 every single month, with a diagnosis of "chronic pain and emphysema" for the past 3+ years, verified by her pain-clinic doctor, who writes all four scripts for her every month. She doesn't seem to have any symptoms of emphysema as her lung capacity is quite good when she shouted, screamed and threatened us for an entire 8... Anybody here dealing with low back pain?
I've had chronic lower back pain for about a year now. It's muscle related for sure, and every morning I wake up I have a tight back and tight hamstrings. Pain subsides with good sleep or after exercise, but really flares up when I'm low on sleep or bending too much. I'm 25 btw. My dream is to go in to surgery and I really hope my back pain doesn't interfere with my ambitions. Has anyone dealt with chronic back pain? Anyone know any surgeons who deal with it? Med School Admission with Disabilities (Chronic Orthopedic Conditions)
Hi Everyone, I am considering applying to medical school but am wondering how physical disabilities affect the admissions process. I have a ruptured/torn L5S1, retrolithesis, and sacroillitis (and a torn hip labrum, internal/external snapping hip and FAI which I hope to resolve with surgery in the next couple of months). PT tech for OMM
Does anyone know if the NMM doctors use any type of tech to speed up their clinic/make it more efficient. It seems like this would be the only way to make it a viable practice model. Arguing for Gen Chem 2 grade (Attendance-Related reason)
At my university, attendance to the gen chem 2 lecture counts for 6% of our grade. Attendance is checked via ungraded 'quizzes' (which is usually a question like, 'how was your day?') which happen infrequently and without notice. Pediatric Pain Medicine Fellowships
Anyone have any insight into this field, it seems relatively new with only afew institutions offering the fellowship. Do you receive a general certificate as a pain physician, or are you limited to only pediatric pain management? What is the caseload? Terminal pediatric patients? Procedural heavy? I love working with kids but I'm wondering if there is a similar lifestyle to adult chronic pain practice? - important to consider every aspect of the field before committing to it. Pancreatic sufficient, but losing weight
Quick recap: My DS, 12 years old, was diagnosed with CRMS because of one mutation & one variant (DF508 & 5T/12TG) and borderline sweat tests (40/41). He has chronic constipation and wasn't gaining weight steadily on a growth curve so his Dr prescribed enzymes as a "trial" to see if they would help. He did begin to gain weight but I couldn't help but wonder if he was just catching up with a normal growth spurt or if the enzymes were actually doing something. The Dr that started him on enzymes left her practice and the new Dr agreed that we could do a little testing to see if he actually needed them or not...
