Breast cancer; prior lymphoma radiation
I have a case of an otherwise health 50 ish year old with a T1 luminal A breast cancer. MRI breast staging confirms T1N0M0. Prior radiation for Hodgkin's at age 15. I know the boards answer here, but it's so tempting to consider this for APBI if she desires BCS. I think cosmestically, etc it'll be fine. I guess my biggest worry is just lifetime risk of further breast cancers. So I'm leaning mastectomy(ies) here. Thoughts? (Source: Student Doctor Network)

Alternatives to a PhD at a university with a very small MD/PhD program
I recently got into a medical school with a very small MD/PhD program. They take about 1 student a year. I'm still on the alternative list for some MSTPs but will likely matriculate into this school. With that being said, I was wondering what other research opportunities, outside of getting a PhD at this school, could be an option. I know that the NIH does a couple internships. Does anyone have a list of opportunities? I'm mainly interested in cancer and clinical research. Anything helps! (Source: Student Doctor Network)

Case: Esophageal Cancer with weird nodes
Hello! I have a case for you and looking forward to hearing your input. Male patient in his sixties, known history of tobacco use. Now presents with swallowing difficulties and is diagnosed with a cT3 G3 SCC of the thoracic esophagus. No immediate regional nodes affected, but two findings on PET-CT: one node in the uppermost mediastinum (almost SCV) and a few other nodes grouped together in the coeliac region. The first biopsy of the tumor was negative and compliance was an issue, so it... Read more (Source: Student Doctor Network)

OMS1: Wondering about heme/onc fellowship competitiveness/planning for the future.
I was wondering about my chances of matching into heme/onc later. From what I can tell, Heme/Onc is competitive but I don't know where it fits on the scale of other residencies/fellowships. is it like: Gastro>General Surgery>Radiology>Heme/Onc>Anesthesia>IM>Neurology>FM I'm only OMS1 right now though at a NEW DO school. Though, before med school, I was pretty set on research and industry. I wanted to do independent cancer research and my PI encouraged me to do med school. I did research... WashU vs. Emory vs. Northwestern
Hello everyone. I am very grateful to have these options, but I am having a difficult time making a decision. I will be an MSTP student, so cost is not something I have to consider (other than COL). In the future, I hope to match to a pediatric residency with a research track, ideally on the West Coast. WashU St. Louis Pros: Very strong in my research interests (cancer immunology). Plenty of labs I could see myself joining Match list is very impressive MSTP administration... Read more (Source: Student Doctor Network)

Hospice or cancer care?
Hi all, I realize this is a good position to be in, but I need help making a decision. I’ve been accepted to a summer research program at a fairly prestigious medical school where my PI has informed me that if I put in good work, my name will put on a manuscript or two. Similarly, I was accepted to a competitive volunteering program (affiliated with the same university) to volunteer in a cancer care center. It would be an excellent experience and I would be volunteering in a variety of... Read more (Source: Student Doctor Network)

Controlled Rxs- fill or refuse?
Hey everyone, When do you feel it's appropriate for a patient to be on high doses of Norco, oxycodone, lyrica, modafinil and xanax? Considering it's not a cancer patient and the doctor has been disciplined in the past by the medical board for over-prescribing. Would you keep filling these prescriptions? Or how would you go about refusing to fill them? What would you say to that regular customer and doctor? Do you often see people being on 2 short acting opioids? I'd appreciate your input... Rad Onc in The Lancet Oncology
Primary radiotherapy and deep inferior epigastric perforator flap reconstruction for patients with breast cancer (PRADA): a multicentre, prospective, non-randomised, feasibility study There are a number of trials in this space now looking preoperative vs. postoperative radiation. I think this is the foot in the door to definitive radiation for selected pts. If you can treat positive nodes definitively with RT, you probably can do the same for (some) primaries (Source: Student Doctor Network)
